Operator voluntarily disconnected and removed hose from area of 3-bay sink at the time of inspection - corrected. 8A - Food and boxes of single use utensils found stored on floor in kitchen. 11C - Refrigerator and upright freezer interiors grimy and food debris observed. 12C - Hot water faucets in sinks in women's restroom and left bar sink observed leaking. 15 A - Floors found not smooth and easily cleanable in kitchen ( missing floor tiles), bare concrete floors in dry storage area, walkin cooler, and behind bar ( repeat ). 15 A - Mold observed on walls of walk-in cooler 15C - Kitchen area cluttered with unnecessary equipment and numerous unused plastic containers with several found in the sink of the hand washing station restriciting access.
