11C - food contact surfaces not washed or sanitized between uses - observed a dough knife used to scrape anything off surface between uses but nothing else was done and the dough knife was then used for the next item
12D - toilet facilities dirty and do not have a self-closing door or closing garbage can lid
12E - no single hand towels at 3-bay sink for handwashing-bathroom sink is inaccessable without the increased possibility of re-contaminating already cleaned hands
14B - effective measures not used on outside storage shed to revent rodent entrance into shed - all around shed door opening is many harborage sites for rodents due to piles of garbage/waste items which are not appropriate for the dumpster |
