THE FACILITY HAS REPAIRED THE PLUMBING IN THE BASEMENT. THE FACILITY HAS WAHED, RINSED, AND SANITIZED THE ENTIRE BASEMENT. THE FACILITY HAS REMOVED EVERYTHING THAT WAS PRESENT IN THE BASEMENT AT THE TIME OF YESTERDAYS INSPECTION. THIS MATTER HAS BEEN REFERRED FOR ADMINISTRATIVE ACTION UNDER CASE NUMBER PHP-6351-16-MK-VAST-FSE WITH A HEARING TO BE HELD ON MAY 12, 2016 @ 930 AM TO BE HELD AT 25 MOORE AVENUE, MT KISCO NY 10549
