At this time observed numerous recurring violations. Notice of Hearing issued for presence of rodent droppings throughout facility: PHP-6308-16-MK-NMB1-FSE waiver option to be considered if ALL above violations are corrected by January 28th, 2016. Remove ALL personal items, excessive cardboard and plastic bags, and black milk crates from kitchen, ALL surfaces to be accessible for cleaning, items such as cleaners, oil containers, pans, etc. to be stored 6-inches above floor. **ALL FOOD PREPARATION TO TAKE PLACE AT FOOD PREP SINK. FOOD PREPARATION AT 3 COMPARTMENT SINK IS NOT PERMITTED.
