10/19/2015Re-InspectionFSE operating without permit which expired on 8/31/15 As part of stipulation for PHP-4154-14-MK-KAA2-FSE,employee required to take Serv safe course by May 2015. PHP-6201-15-MK-KAA2-FSE issued at this time for failure to renew FSE permit within 60days of getting renewal application in the mail,operating with an expired permit and violating BOH order by not taking serv safe course within 60 days of receiving order and not paying assessed civil penalty of $3400 FSErenewal applicaton still incomplete, missing workers compensation and disability insurance documents. KItchen ordered shut at this time until the permit issue is resolved Re-inspection scheduled for 10/26/15,correct all violations immediately
