  • payment was made 6/29/15; will expire 6/16; establishment needs to make sure and post most current food permit in a public view.

  • Make sure to remove all canned products that have a dent anywhere on/around the can. Also go through produce and remove all bruise apples and bell peppers not in sound condition. In addition, remove packaged soda cans/other canned beverages that have spilled/leaked.
  • Establishment will need to limit the presence of gnats near where spilled izzie apple drink/other packaged drinks are located. 229.167 (p) (11) (C)insects/pests not minimized.
  • Will need to provide most current food permit/receipt showing payment to be posted; management checking on the status of this.

  • Many sound condition issues during inspection. Observed many dented canned products on shelves; opened bag of hot cheetos on shelf next to other bags; highly bruised apples observed; ice cream within reach in freezer that was melted; management in the process of removing melted ice cream products from freezers, removing bruised apples, and going through shelves to check on dented canned products.
  • Many gnats/fruit flies around produce within storefront; will need to use approved pest control measures to eliminate this problem; pest control(terminix hasn't come out this month yet). 229.167 (p) (11) (C)insects/pests not minimized.
  • Spray bottle within cardboard box near produce without any label as to what is inside.
  • Need to make sure and post most current health report; current food permit was not posted.
