  • Employee in kitchen kept touching her face, dirty apron, and other misc. items then handling single service items and deli paper and other employee continuously stroked hair over ears. **All employees must properly wash hands after any activity that contaminates hands.
  • Open employee drinks stored above disposable cups and on top of ice machine. **Do not store employee drinks next to food or any items that need to be protected from contamination.
  • 4 bottles of mixers in ice that was served to customers and cutting board/knife was stored behind faucet at warewash sink in dirty water. **Discontinue using ice for consumption.
  • No backflow preventer installed at threaded faucet for mop sink. **Install backflow preventer or install aerator.
  • No proof of food manager certification. **Provide proof of certification.
  • Ice machines has visible mold, thermometer in dirty water coated with food debris, and can opener filthy. **Clean and sanitize all utensils and other food contact surfaces.
  • Employee picked up sliced tomatoes with barehands and put into a container and no documentation is provided as specified for this contact. **Use physcial barriers or provide documentation as specified.
