- Observed food debris and plastic in the hand sink. Ensure that the hand sinks are only used for hand washing. Items may not be emptied in the hand sinks, all waste must go into the trash can or mop sink.
- 3 compartment sink was set up incorrectly. Employee stated the 1st compartment had soapy water and middle compartment had chlorine solution, but chlorine was not detected. Ensure when ware washing the 3 compartment sink is set up as follows: 1st with hot soapy water, 2nd for rinsing, 3rd with sanitizing soultion (if using chlorine, the concentration needs to be 50-100ppm).

- Observed employee drink bottles stored in walk in cooler near food. All employee drinks must be in a cup with a lid and straw and stored ini a designated area away and below any food that is going to be served to customers.
- Observed raw bacon being stored above ready to eat foods in the walk in cooler. All raw animal products must be stored below and away from ready to eat foods. Also observed foods uncovered in the walk in cooler. Ensure that all foods are covered when being stored to prevent cross contamination.
- Foods in walk in cooler did not have any date labels. All potentially hazardous foods that have been cut and/or cooked must have a use by date of 7 days, the date prepared counting as day 1.
- Observed a cloud of gnats in the dry storage closet surrounding a dirty piece of equipment. Ensure that measures are taken to reduce the presence of pests.
- Observed several containers stored on the drying rack across from 3 comopartment sink with flour and food still stuck on them. Ensure that all dishware/containers/utensils are properly cleaned and sanitized before being stored on rack to be reused. Drying rack needs to be cleaned of all food debris as well.
- Observed a piece of equipment in the dry storage room with food debris still stuck on it. All items that are not being used must be properly cleaned before being stored to prevent any pests issues.

- 229.168 (a) toxics not labeled. Observed toxic chemicals in storage room not labeld and blue mixture had a Bill Millers conatiner floating inside used as a scoop. All cleaning chemicals must have label or common name and used according to manufacturers label.

- 229.163 (h) (6) wash as needed. Observed employee leave the kitchen area, then return to the area and not wash hands. Manager had to advise the employee after walking towards the food line to wash her hands. Food employees not cleaning their hands and exposed portions of their arms as specified immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks. Employees should wash hands when changing tasks or leaving an area and also periodically to ensure proper hand washing is being enforced.
- 229.168 (a) toxics not labeled. Pink soap inside bill millers containers above the 3 compartment sink not labeled...all chemicals should be labeled.
- Clean dishes were being stored on a dirty rack. Utensil were seen in a plastic bin and appeared to be washed but still had food debris on the surface. Rack and utensils should be clean and free from food debris.

- 229.163 (n) (1) eat.drink.smoke.. Personal drink with no cover or straw, on food prep-table. All personal drinks must have a lid and straw, and do not place them on food prep-tables. Also, observed employee use handsink, to dump out her personal drink. Handsinks are only for hand washing only and nothing else.
- 229.164 (e) (1) (D) (ii) (V) doc- other methods. Observed employee with long finger nails, handling the onions and tomatoes, and placing them in packages with bare hands. Instruct employees that when handling RTE food, with bare hands, they cannot have long fingernails.
- 229.167 (p) (11) (C)insects/pests not minimized. During inspection, employee was taking out trash, and left back door open. While door was open, observed several flies, enter kitchen area. Instruct employees to close door behind themselves when entering and exiting from back door.
- 229.168 (a) toxics not labeled. Found unlabled chemical being stored on top rack of drying rack, directly above cleaned dishes. All cleaners must be labled with product information, and do not store them in manner, where they may contaminate dishes.
