229.167 (e) (2) no soap. Observed broken soap dispenser with no soap in kitchen area. Manager stated that new soap dispensers are being installed. Ensure soap is available at all times at every hand washing sink.
229.163 (h) (5) handling soiled equip. Observed employee handle dirty dishes then stack clean dishes without washing her hands. Food employees must clean their hands immediately before working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and after handling soiled equipment or utensils.

229.165 (d) (1) (E) (i)not easily cleanable. Observed rusty blade on fruit slicer. Ensure blade is kept clean and rust free. Change blade when it becomes rusty.
229.165 (m) (1) (A)not clean. Observed many plates and bowls stored with clean dishes that had food debris present. Ensure all dishes are clean to sight and touch before storing with clean dishes.

229.164 (v) (5) can foods - damaged, etc.. Observed some heavily dented cans stored with cans in sound condition. Ensure all heavily dented cans are stored separately from good cans and are returned to vendor or discarded.
229.164 (v) (4) PHF > shelf life. Observed tomato with mold growth stored in cold hold unit. Ensure all phf is discarded once mold/mildew growth can be seen.
