- There are numerous flies inside the facility. There are numerous dead cockroaches along the base cove and underneath the counter. Observed one live adult cockroach fall onto raw meat product inside the meat case. Item discarded. Remove all dead cockroaches and maintain pest control. Employee states facility is serviced 2-3 times a month.

- Details:
- Some of the meat cutting machines still had some meat residue. Clean machines completely when not in use.
