- Shelve under steam table is not clean, and metal rack next to ice maker has residue. Detail clean as often as necessary. Correct by 2 days. Caulking behind ware washing equipment is falling off and is black with mold. Remove old caulking and replace by 2 weeks.

- Employee rest room has some unsanitary surfaces. Shall be cleaned more often.
- Comment:
- Toilet facilities shall be maintained clean, sanitary and in good repair. Toilet rooms shall be separated by a well-fitting self-closing door. Toilet tissue shall be provided in a permanently installed dispenser at each toilet. The number of toilet facilities shall be in accordance with local building and plumbing ordinances.
