- Cool down times were on charts but only dates on products. Record dates and times of onset of cool down on products, too. Corrected during inspection.
- Paint or seal the rusty shelves in the kitchen walk in cooler.
- Replace the damaged door seal on the Victory cooler in the kitchen.
- Replace the damaged and missing floor tiles in the middle of the kitchen and under and around equipment.
- Repaint the wall above the kitchen hand sink where the paint is peeling.

- Observed cooked and cooled meatballs, pork, beef in the kitchen walk in cooler that were only labeled with the date. These foods were at or below 41F. Label with both the date and time of onset of cool down.
- Observed unlabeled cooked and cooled ribs, meatballs, wild rice in the Victory cooler in the kitchen. Observed unlabeled mushrooms stuffed with cooked sausage in the small prep cooler in the kitchen and unlabeled cooked and cooled ribs in the kitchen walk in cooler. They must be labeled with both the date and time of onset of cool down. Manager discarded the above foods.
- Observed uncovered ribs and garlic bread in the Victory cooler in the kitchen and uncovered bulk chicken base and sugar on the shelf in the dry storage room. Cover foods to prevent contamination during storage. Corrected-- manager covered foods.
- Observed an employee wash their hands and then turn off the faucet with their bare hands, recontaminating their hands. Employee was retrained on site by inspector. Manager retrained all employees on proper hand washing procedures at the time of the inspection.
- Repair or replace the white chest freezer in the back kitchen with exposed insulation hanging from it. Repaint the chipped shelf under the boiler in the back kitchen area and rusty shelves in the walk in cooler in the kitchen. Remove cardboard lining stainless prep counter in back kitchen with tubs of batter mix on top of it (corrected).
- Replace damaged cooler door seal on the Victory cooler in the kitchen.
- The insides of the coolers and freezers throughout the facility had a build up of food debris and the can opener blade was soiled. Clean and sanitize these areas.
- The pizza oven in the back kitchen area had a build up of food debris. Manager stated they no longer use it and he will dispose of it.
- The outsides of the microwaves and cooler and freezer door handles and shelves in the walk in cooler had a build up of debris. Clean and sanitize these areas.
- Repair or replace the leaky faucet at the 3-compartment sink in the back kitchen area and thoroughly clean the dirty mop sink.
- Repair or replace the damaged kitchen floor tiles in the middle of the kitchen and under and around equipment. Remove and dispose of cardboard under fryers. This is not an approved flooring in a food establishment.
- The kitchen and walk in cooler floor had a build up of debris under and around equipment and shelves. Clean the floors.
- The walls and ceilings in the kitchen and storage area had peeling paint. Repaint them. The ceiling in the dry storage room had some holes near the mop sink with exposed insulation. Repair them. Replace damaged/black caulk around the mop sink and dish machine. Caulk or seal holes in the wall in the mop sink area.
- The walls and ceilings in the walk in cooler, kitchen, dish washing area, storage area had a build up of debris. Clean the walls. Clean the dusty vent in the dry storage area.
- No food handler documentation was available on site.. Per manager, he has misplaced them but all employees were trained on site last year by Sean Farris with the Tazewell County Health Department. He will contact our office about getting replacement copies of these certificates by no later than 1/31/16.
