- The dress table was not maintaining below 41f at time of inspection. When I initially took a temperature it was 51f. The unit was closed and after more than 10 minutes it was 48f. Temperatures were taken at 11am and it was 41f and below. Extra meat was taken out and put in the walk in cooler. Discussed time instead of temperature with tomatoes and dressings. The seal is broken on the right door of the unit which might be contributing to the temperature issue. In the temperature documentation for February there are temperatures for that unit that are 44,45f. Do not use this unit until it can maintain 41f or below.
- The facility's digital thermometer was not working properly. When put in an ice bath it would go from 38f to 50f. It was always changing temperature, but mostly staying in the 50f range. A regular metal stem thermometer was put in the water and read 32f. I told employees to use this thermometer.
- Observed employee improperly washing their hands, he touched the handle of the faucet with his hands instead of a paper towel. I explained how to properly wash their hands using the paper towel as a barrier. Make sure all other employees know how to properly wash their hands. I left a hand washing handout.
- Observed employees not wearing effective hair restraints.
- There is cardboard on the tomato shelf in the reach in cooler in the room with the 3-compartment sink. The seal of the right door of the dress table is no longer in tact.
- There is food buildup at the bottom of the dress table. There is grease and buildup on the grill, fryers,chip area, handles of reach in cooler/freezer in prep area.
- There is buildup on floors especially around equipment and grease accumulation under the fryers.
- There is black buildup on the wall/seal behind the 3-compartment sink. There is grease and buildup on the walls behind the mini flat top, behind and around the steam table, behind the chip area. The ceiling of the walk in cooler with meat is made of insulation and has a black buildup on it.
- There is cardboard used as a ceiling tile over the flat top.
- There is a missing light shield over the 3-compartment sink.

- Found container of raw chick breasts stored above R-T-E foods. Food Mgr moved chick breasts to bottom shelf. Corrected.
- Found bare wood 2X4 holding up counter in food prep kitchen. Also found bare wood trim above other counter in food prep kitchen.
- Found moist wiping cloth on kitchen counter.
- Found grease/scum buildup on overhead grill filters.
- Found dirt/grease/scum/lent buildup on some kitchen shelves, eqp, food containers, fan cover,etc.
- Mens restroom door did not close properly. Door spring was tighted/corrected.
- Found side door to outside patio blocked open. Door was closed.
- Found dirt,grease/scum buildup along perimeter of floors in both kitchens.

- Observed a dirty knife stored on the magnetic knife holder. Please clean utensils before they are stored away.
- Observed dust built up on the fire suppression system above the heating station for chips/ fries. Also observed dust built up on x-mas lights above food trays and metal shelf above food trays. Please ensure non-food contact surfaces are clean to avoid dust contaminating food.

- Observed employees cooking with no hats on. Spoke to employees.
- Non food contact surfaces in disrepair: the counter in prep kitchen is worn, the wall mount to the spray hose for three comp sink is not attached, reach in cooler has rust on shelves, replace black caulk line behind three comp sink. Non food contact surfaces shall be in good repair.
- Chip containers have excess of grease build up on them. Spoke to employees.
- Non food contact surface cleaning: clean hand area of all doors in facility, clean the vents on the back side of the fryers, clean lime off the side of the ice machine in the tiki bar, clean as needed.
- Found dumpster lid open. Corrected and spoke to employees.
- Found excess of garbage on ground by the dumpster area. Throw garbage bags and garbage into the dumpster.
- Found door to outside seating area propped open. Corrected and spoke to employees. Fan was blowing out door so no flies would enter.
- Provide light shield to light above three comp sink and by ice machine in the basement storage area.
