- Observed rusty shelves in the walk in freezer in the kitchen. Also observed rusty bars in the beverage cooler in the bar area. Spray or repaint.
- In kitchen, observed broken gaskets on prep cooler door closest to hand sink and kitchen entrance. Replace.
- Observed debris build up in the following areas: crates used for potato and onion storage in the back area of kitchen; in the freezer by kitchen fryer, on nozzle holder of beverage dispenser and nozzle of beverage dispenser in bar area. Clean. Also observed extensive build up of debris in ice machine in kitchen and bar area. Recommended discontinuation of use, dumping of ice and extensive cleaning of the inside of both machines.
- Observed debris build up in the following areas: on the dough mixing equipment, side of fryer and grill in kitchen, all shelving units in the kitchen, cart by fryer in kitchen, in cabinet under coffee machine and hand sink in the general sitting area of the facility. Clean
- Observed absence of sink in kitchen bathroom. Makeshift sink had cardboard boxes stored in it. Recommended discontinued use of kitchen bathroom until problem rectfied.
- Hand soap and paper towel were not observed in the kitchen bathroom.
- Toilet bowl in kitchen bathroom were dirty.
- Observed debris build up on floor in inner walk-in freezer in back area of kitchen. Floor behind fryer/grill and under shelves especially under the 3 compartment sink and under shelves in the general back storage area. Clean floors.
- Observed pesticides stored (with tools and miscellaneous items) on ice machine. The cans were immediately removed by facility owner.

- Found 2 badly dented cans of cheese sauce on shelves. Items voluntarily disposed of.
- Found opened bag of hot dogs without date label. Please label all potentially hazardous food after opening.
- Cold holding unit across from pizza does not have any shielding over food. Please cover food at all times.
- Observed open cardboard box of glasses being stored on bar floor. Please ensure all food contact surfaces are stored 6
- Observed damaged door seal gasket on walk-in cooler. Also observed warped/ damaged wood above 3-compartment sink in bar.
- Observed spill on bottom shelf inside juice/ mixer cabinet at bar. Please clean.
- Observed black build-up in ice machine inside exposed mechanical unit. Please clean.
- Hand sink in bar does not have means of soap or paper towels. Please provide soap/ paper towels at all hand sinks.
- Observed light fixture above (out of service) ice machine in kitchen without proper shielding. Please provide shielding.
- Observed weed whacker stored in kitchen. Please remove unnecessary items from kitchen.

- FOUND TEMP OF SLICED TOMATOES, COOKED GYRO MEAT & ITALIAN BEEF 48* IN FRONT FOOD PREP COOLER. COOK VOL DISCARDED ABOVE FOODS.
- FOUND OPEN COMM PACKAGED ITALIAN BEEF & MEAT BALLS IN WALKIN COOLER WITHOUT 7 DAY DATE MARKING. ALSO FOUND SEVERAL CONTAINERS OF PREVIOUSLY FROZEN FOOD WITH NO NEW DATE THAWED.
- COOK DID NOT HAVE BEARD COVER ON.
- NO LID ON ICE MACHINE AT BACK BAR.
