- Observed opened cans of food (corn, pineapple, and corn beef hash) stored in the prep cooler without date labels. Manager voluntarily discarded these food items.
- Found food stored in uncovered containers in the prep cooler across from the grill. Cover food during storage.
- Observed staff handling/prepping food without hair restraint. Use effective hair restraint.
- Observed used paper towels and debris stored on clean utensils. Manager decided to discard.
- Observed rusty shelves in the kock equipment. Replace rusty shelves.
- The following equipment had buildup of debris in them: the prep cooler across from the grill line (including the door gasket and the containers holding food on the upper cold holding part of the cooler) the fryer, the microwave across the prep cooler, the toaster across from the grill line, the shaker machine on the shelf across from the grill line, the hot point fridge/freezer, and the true cooler. Also the whirlpool and gibson freezer and the white chest freezer beside the fryer need cleaned and defrosted. clean.
- The following equipment had buildup of debris on them: the outside of the hot point fridge especially the door and handle area. Clean. Shelves in the utility room by the kitchen have debris buildup. clean.
- Observed debris on kitchen floor, especially under, around and behind equipment. Also in corners. Clean floor.

- Found container of ham & beans, broccoli soup, bean soup and turkey gravy stored in the 2-door reach in cooler cooled down with no date/time of removal from heat. Cook discarded foods. Facility not allowed to cooled while being a CAT II risk.
- Observed small dish being used to scoop chicken batter. Provide proper scoop utensil.
- Found duct tape on the corner of the 3-comp sink; Removed. Found broken door seal gasket to the left door of the 2-door prep cooler; replace. Observed rust on various reach in cooler/freezer shelves; recoat/replace shelving. Observed dining booth seating torn with padding exposed; repair.
- Found dishes under prep counter soiled stored as clean. Wash, rinse and sanitize.
- Observed most kitchen equipment have build-up of food debris or grease. Facility shall wash, rinse and sanitize all kitchen surfaces (storage racks, cooler/freezer door seal gaskets, sides of oven/fryers, cabinets). Observed exposed wood throughout kitchen; remove exposed wood or seal wood so it's non-absorbent easily cleanable.
- Found drinking cups stored in point of sale are stacked together still wet. Allow cups to be air dried.
- Found wall/floor rubber cove molding in disrepair; repair.
- Observed food debris and grease build-up on the floors behind the store/fryers. Clean floor.
- Observed walls with grease build-up and the kitchen light shields with food splatter build-up. Wash, rinse and sanitize.
- Observed white wall paneling in disrepair next to the prep cooler and some ceiling panels in disrepair. Repair.
- Observed mop bucket stored next to hand sink, not in use. Properly store bucket in mop sink room.

- Found containers of ingredients not labeled. Items outside of their original container must be labeled with common name of food/ingredient.
- Found packages of opened cheese in prep cooler near grill. Foods must be labeled once they are opened with the date it was opened or the date it must be discarded.
- Found several items uncovered in the reach in coolers including corn, iced tea, and fish in the reach in freezer. All items in storage must be covered at all times.
- Employee was not wearing any hair restraint while in kitchen prep area. Hair restraints must be worn when cooking and/or prepping food.
- The prep cooler is missing a handle. Replace. One of the shelves in the reach in cooler is cracked. The shelf above the hand sink is peeling. Repair.
- Found no sanitizer in bucket on counter behind point of sale station. Employee dumped water and remade new bleach. Concentration was 100ppm when fixed. Routinely check sanitizer concentration throughout the day.
- Found food and grease build up on the grill and on the sides below the grill. Wash, rinse, and sanitizer the grill.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles and cupboards throughout the kitchen, cooler and freezer door seal gaskets, cabinet fronts including for the utility sink area, the inside of the condiment
- Found cups in the point of sale area that were stored wet. All items needs to be air dried prior to storage. Rewash, rinse, and sanitize cups.
- Found lemonade stored in the cooler in a reused milk jug. Single use items cannot be reused.
- Faucet for the 3-compartment sink dripped for several minutes after it was turned off. Repair the sink.
- Build up on floors especially under and around equipment. Clean the floors.
- Ceiling panels are bowing or are not in place. Repair panels.
- Seal for 3-compartment sink is almost completely chipped away. Replace the sealant.
- Stained walls throughout the kitchen area. Clean the walls.
- Found mop stored in the bottom of the bucket. Store mop free hanging from a wall or in the top of the bucket so that it may drain through the slots.
