- Observed the following foods without date labeling: chorizo, beans, cream based sauce, and unknown maroon colored item. Manager disposed of all items. All items made with potentially hazardous food must be labeled with a 7 day date labeling. Provide TCHD with training documentation for date labeling by 11/16/15.
- Observed individuals in kitchen without hair restraints. All employees handling food must wear effective hair restraints.
- Observed damaged hotel pan, hotel pan lid, and cutting boards with abrasions and discoloration. All food contact surfaces must be in good repair. Replace or repair.
- Observed damaged handle on kitchen 2 refrigerator. Repair.
- Observed bar hand sink was blocked by a strainer filled with straws and food debris. Hand sinks must be accessible at all times and only used for handwashing. Manager removed strainer during inspection.
- Observed the following damage to the floor: in front of the stove line has cracked and peeling floor covering, and peeling yellow paint on step going into kitchen 3. Repair to allow for proper cleaning. Observed unfinished wood on floor in bar storage area. Finish or replace wood to allow for proper cleaning.
- Observed the following areas of the floor to have a build up of debris: drain below dish machine, under/around ice machine, under equipment, water heater pan. Clean as routinely as necessary.

- Observed pink and black build up on ice drip tray. Wash, rinse, and sanitize.
- The back hand sink in back prep area was out of paper towels. The bar hand sink was out of soap and paper towels.

- Containers of ingredients not labeled. Bulk ingredients outside of the original container need to be labeled.
- Two clothes in room next to bar area with cloths outside of container. Store wiping cloths in sanitizer.
- Observed dark spots on ice machine drip tray. Wash, rinse, and sanitize drip tray.
- Non-food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the top of the reach in freezer, freezer door seal gaskets, and the bottom of the freezers/coolers.
- Dust build up on floors especially near equipment. Clean the floors.
- Wall near doorway to kitchen has chipped paint and some of the wood is showing through near the bottom of the doorway. Repaint and reseal doorway.
