There is cooked and cooled down sloppy joe in the freezer without a time of onset of cool down.
Observed an employee improperly wash her hands then proceed to handle ready to eat foods with her bare hands. I also saw another employee improperly wash his hands. They both touched the handle of the faucet with their hands and not a paper towel. The paper towels aren't close to the hand sink in the front of the facility which may contribute to it. Per manager they're getting new paper towel dispensers. You will need to conduct a training and verify all employees can properly wash their hands. You also need to use gloves.
Observed missing ceiling tiles and exposed insulation on the walls in back dry storage room. The paint on the wall behind the 3-compartment sink looks like it's starting to come off.
The temperature charts are not current. You must complete 2 weeks of temperature charts and submit them to me.

The chef that was just hired was wearing a rubber bracelet. Explained that per code they can only wear medical jewelry and a plain wedding band.
Employees drink from water bottles. Explained they need a hygienic container that will not contaminate hands, preferably a cup with a lid and a straw.
There is laminate missing and coming off on the side of the hand sink by the 3-compartment sink. Explained this is a sink/vanity meant for the home and not commercial grade. The seal is broken on the reach in cooler that holds deli meats. The trash receptacle in the soda fountain area is missing laminate. There is parchment paper used under pans in the prep area. Parchment paper should not be used as a substitute for cleaning.
The ice machine has buildup in it.
Observed an employee towel drying food contact surfaces.
Observed single use items being used in the facility.
There is construction going on at the outside of the facility towards the parking lot. Currently by the basement there is plywood separating the outside to the kitchen. There is gaps on the top and bottom of the plywood. There is a piece of tarp to help prevent contamination, but it was open at time of inspection. Construction of this area must be corrected within 30 days.

Observed employees not wearing effective hair restraints.
There is cardboard under the slicer. The cardboard is dirty and stained. There were rags in a couple places in the prep area including covering a pipe by a block of wood. Both the rag and wood aren't easily cleanable. I pointed this out to the manager. The seals to the reach in cooler with the fruit and pancake batter are broken.
The buckets used to store utensils have food debris on the bottom. Also the ladles have accumulated debris due to the way they are stored. The milkshake mixer had buildup on it as well as the small mixer.
The facility reuses single service storage containers (cottage cheese) to store cooked and cooled down foods such as soup. Explained these are not meant to be reused.
Observed gap in the middle of the door to outside at the front of the facility. I pointed this out to the manager. Door must be repaired so no daylight is visible by 3-18-15. Contact me once repaired.
There is a missing ceiling tile over the coffee machine. Also on the wall by the 2 reach in coolers there is a hole in the wall with wiring. The wall behind the 3-compartment and knife storage has paint coming off.
The storage room in the back is full of items that have not been used in some time. Discussed with the manager going through these items and if they aren't going to be used getting rid of them. Also decreasing if you have several of one thing (roasters).

Found lunch meat that was not labeled. Ensure all ready-to-eat potentially hazardous foods are labeled after opening and follows the 7 day rule.
Found food items in refrigerator and freezer that have been cooled and do not have cool down documentation. Ensure documentation of all cooled down foods are maintained. Provide cool down chart/documentation for next ten days of operation to TCHD by 1/13/15.
Observed employees washing hands incorrectly. Ensure paper towel is used to turn off faucet.
Found 2 sanitizer buckets with chlorine solution. Both sanitizers were 0ppm sanitizer. Maintain chlorine sanitizer in buckets at 100-200 ppm. Sanitizer concentration was adjusted to 100 ppm at time of inspection. Sanitizer test strips were sold to facility at time of inspection.
Found plastic cups stacked together wet. Ensure dishes are thoroughly air dried before stacking.
Found missing ceiling tiles in kitchen. Repair or replace.

Observed employees not wearing effective hair restraints. All employees working with food must wear hat or hair nets or other hair coverings.
Replace dirty particle board blocks raising up the 3-compartment sink with ones that are non-absorbent and easily cleanable. Observed damaged door seal gaskets to the sandwich make table in the kitchen. Per manager, the seal has been ordered already and they are waiting for it.
Wait staff wiping pail was at less than 50 ppm. Maintain 100-200 ppm. Corrected to 100 ppm.
The inside of the wait staff cooler had a build up of debris. Clean and sanitize it.
Stainless pans were stacked wet on the clean dish shelf.
Repaint chipped wall around the soap dispenser in the men's restroom.
