Observed cooled down foods without time of onset of cool down.
In the 4-door reach in cooler in the basement there was beef including pre made hamburger patties stored over shrimp.The shrimp was moved.
There was a stained rag at the bottom of a drawer containing utensils in the prep area. Discussed with owner limiting use of rags.
There is a leak at the bottom of the handsink across from the stairs.
Observed medication such as Ibroprofen stored with food such as brown sugar. The medication was moved.
The facility didn't have any current temperature documentation. Per kitchen manager he had taken home temperature charts for the coolers/freezers to make a copy. The facility isn't currently completing cool down charts. Discussed documenting cool down. I need to see 2 weeks of completed temperature documentation including cool down by 12-14-15.
