  • Observed uncovered food (pulled pork and ice cream) stored in containers without lids in the walk-in freezer. Cover food stored in coolers/freezers.
  • Observed broken gaskets on doors of the prep coolers in the kitchen. Replace.
  • Observed debris build up in the following equipment/areas: prep cooler gaskets in the kitchen (cooking area), inside of the Pepsi cooler in the kitchen, inside of the Aquafina cooler (especially the shelves and door). Clean. Observed debris build up on dish washing crates by dishwasher. Clean.
  • The Pepsi cooler has debris on the outside. Clean.
  • Observed wet pans stacked wet in storage on shelf by the second hand sink in the kitchen. Rewash, rinse, sanitize and air dry prior to storage.
  • Observed buckets blocking hand sink by ice machine in kitchen. Remove buckets to make hand sink accessible for use. Buckets were removed from the front of the sink during this inspection.
  • Observed wet floor in kitchen. Per manager, the floor was being cleaned, but there was no sign of anyone cleaning during inspection. Keep floors clean and dry.

  • There was cheese in the bottom of one reach in cooler and other items in the reach in cooler that has raw meat. The cook covered them at time of inspection.
  • Tortillas are being stored in to go bags. Discussed that this is not food grade plastic.
  • There was one rag under a cutting board and another in the grill area. They were removed. Per owner they are used to clean then when they get dirty they're replaced. Make sure that they are removed right away.

  • In the walk in cooler there was a container of what appeared to be labeled polio with the date 1-16 or 1-18 and pork 1-21-15. The manager and I were unable to determine the actual date of the container labeled polio if it was 1-15,1-16 or 1-18. Those 2 things were voluntarily disposed of due to being past the 7 days. Also there was a container of refried beans with no date label. The manager was able to determine the date due to temp charts. It was labeled with the correct date.
  • The 2 food items that were past the 7 days were voluntarily disposed of.
  • Observed employee improperly washing their hands. The employee was told to wash his hands and he only used soap on his hands for 5 seconds. He was told to wash his hands for 20 seconds. He did use the paper towel to touch the handles to turn off the faucet.
  • The cook wiped his hands on a rag that was on the counter, used it to clean, then touched ready to eat foods with his bare hands. First whenever you change tasks you must wash your hands. Second you should not wipe your hands on a dirty rag that's being stored on the counter. Third the facility does not have a bare hand policy so you must use gloves and utensils. The box of gloves wasn't even in the prep area. The employee in the dishmachine area did the same thing, he wiped his hands on a dirty rag that was being used to clean then touched clean dishes without washing his hands. Both were told by the manager to wash their hands when changing tasks and to not wipe their hands on dirty rags. You must conduct a training going over proper handwashing and hygiene. Then you must watch every employee properly wash their hands.
  • Also in the dishmachine area the employee had a drink but it was just a cup. The same in the prep area there was a cup with no lid or straw. Discussed with manager that ideally employees should have drinks with a lid and a straw.
  • Observed damaged door seal gaskets on the reach in prep coolers. I pointed this out to the manager.
  • Found wiping clothes stored on counters and on an employee's apron. Store in sanitizer pail between uses.
  • Found build-up on handles of reach in coolers on prep line.
  • The handsink by the mop sink didn't have papertowels.
  • The lights in the basement aren't shielded. There is food and food contact surfaces that are stored in the basement. Discussed getting the bulbs that have a coating so that if they break the glass is self contained.
  • There was a spray bottle on the dishmachine that wasn't labeled. Also the facility uses garbage cans as sanitizer buckets. You must label all chemical containers. The spray bottle was labeled.

  • Found raw chicken stored next to cooked cheese sauce and above raw beef and raw shrimp in the walk in cooler. It was moved to the bottom shelf. Also, found raw steak stacked on top of a covered pan of sliced tomatoes in the walk in cooler. corrected-steak was moved to another lower shelf.
  • Found uncovered raw chicken, cheese sauce, and other raw meats in the walk in cooler and freezer. Found uncovered bulk spices and taco shells in basement storage area. Keep foods covered at all times to prevent contamination.
  • Found plastic and metal containers on clean dish racks stacked wet.
  • Found clean utensils stored with the handles down at the silverware wrapping table. Store with the handles up to prevent contamination.
  • Repair the faucet at the hand sink nest to the salsa cooler so that it turns on and off easily without dripping.
  • Replace missing grout and tiles next to cook line.
  • The chlorine sanitizer wiping pails for the wait staff and cooks line were at >200 ppm. Maintain at 100-200 ppm. Corrected to 150 ppm.

  • Found refried beans in walk in labeled 8-11 with no time at 55F, 46F. Also, found cooked rice (at 45F)and chile renos in the walk in cooler with no label. Label with the date and time of onset of cool down. No temperature documentation was available for these items. They were discarded by manager.
  • Found uncovered foods in the walk in freezer. Cover food to prevent contamination. Found uncovered food in dry storage area (2 tubs of tortilla bowls). Cover food to prevent contamination.
  • Shelves in walk in cooler and small glass-door cooler had a build up of debris. Clean and sanitize them.
  • Clean metal pans and plates were being stacked wet. Allow them to air dry before stacking them.
  • Hand sink was not accessible. Found dishes stored in dish room hand sink. Dishes were pulled out of hand sink and rewashed.
  • Found damaged floor tiles. Repair or replace them so they are easily cleanable.
