Observed thermometer at ~22 F in ice bath. Thermometer should be at 32 F in ice bath. The thermometer was calibrated at the time of this inspection.
Observed food items in the cooler that according to staff are employee items. Ensure personal items are labeled accordingly.
Observed missing outlet cover in dry storage area. Provide outlet cover. Observed towels used as a shelf liner in the bar coolers. Towels are not smooth, non-absorbent, and easily cleanable. Remove.

Found cooked mushrooms at 75°F, cooked onions at 78°F, and Cooked peppers at 69°F in containers on the prep counter. Maintain cooked foods at 135°F or higher. Spoke to manager, she stated that these items are cooked at the beginning of lunch everyday and discarded after 4 hours. There was no written procedure or date and time labeling. These foods were discarded by the manager. She stated they have ordered a new steam table and will keep these items in this unit when it arrives.
Observed raw eggs stored over ready-to-eat foods in the glass-door True fridge. Store raw eggs below all ready to eat foods. Eggs were moved to bottom shelf.
Found a bowl being used as a scoop in a bulk potato chip contained on the counter. Provide a scoop with a handle.
During today's inspection it was noted that your facility is in violation of the food handlers requirement (750.570). This requirement is to be corrected by your next routine inspection. Owner stated the rest of the staff will be doing the training in March 2015.

In the reach in cooler by the grill, observed a container of raw chicken being stored above raw hamburger. Discussed with manager and an employee moved the raw chicken to the bottom shelf.
In the storage closet there is wood throughout such as below pipes and used as a shelf for chemicals above the mop sink. Wood is not easily cleanable. It needs to be sealed. Cardboard was being used on the shelf in the storage closet. Remove cardboard so shelving is easily cleanable.
Some yellow build-up on the outer rim of the drip tray in the ice machine. Clean.
The back door to the outside has a gap at the bottom. This needs to be repaired so that daylight no longer shows. The gap allows pest entry. This needs to be repaired by 12-3-14. Once it is repaired contact Katie at (309) 925-5511 ext. 241

Found grilled onions and peppers that were cooked and cooled down in the reach in cooler. Discussed with manager and food was voluntarily disposed of. Also there was a container of cooled and cooled down taco meat in the reach in beer cooler labeled as personal. If you are going to take home leftovers they must be taken home that day. As a category 2 facility you are not allowed to cook and cool down anything except pasta noodles.
