  • Observed employees not wearing effective hair restraints. Provide hats, hairnets, or other hair coverings.
  • Replace the missing section of ceiling in the bathroom the is adjacent to the kitchen. Replace missing walls and ceiling in the water heater closet in this restroom. Patch the small screw holes in the walls in the kitchen. Seal around the area where the PVC pipe comes through the wall in the back storage area.
  • Found personal medications stored on a shelf next to the 3-compartment sink above bananas. Corrected--it was moved into a drawer with no food by the drive thru window.
  • Employees have not attended a food handler training. Employees must be food handler certified within 30 days

  • Found soda syrup lines and ice cream flavoring lines on the floor.
  • Found single service items stored on the floor in the dining area.
  • There were no self closing devises on the restroom doors.
  • The walls in the back storage room were bare wood. Paint or seal them so they are easily cleanable.
  • The chlorine wiping pail in the kitchen was >200 ppm. Maintain at 11-200 ppm. Corrected to 100 ppm.
  • Found Home Defense bug spray stored with Scrubbing Bubbles and other cleaning chemicals in the chemical supply closet. Manager removed bug spray to be taken home.
  • 750.560 This facility was found to be in violation of the Food Handler requirement. Currently, none of the employees have taken an approved Food Handler course. You will have until your next routine inspection to get all employees without a Food Manager certificate Food Handler certified.

  • In the tall reach in cooler there was an opened package of turkey lunchmeat that did not have a date label of when it was opened. Also in the reach in cooler under the prep table there was hot dogs, sour cream and salsa all out of their original container with no date label. Once a package of lunchmeat or hot dogs is opened, it needs to be labeled with the date and used within 7 days. When you take foods out of their original container you should also label them with the date of removal from the container. Per owner the hot dogs and turkey lunchmeat were taken out on Saturday. Per owner they go through them pretty quickly. The items were all date labeled and verified they are not past 7 days. Also milk carton needs to be date labels upon opening.
  • Observed employee touching ready to eat foods (bun) with what appeared to be acrylic nails. Anyone wearing nail polish or acrylic nails must wear gloves when touching ready to eat foods. The facility has a bare hand policy but that doesn't apply to nail polish or acrylic nails. Discussed hygiene with owner. Owner will need to add something to bare hand policy and train employees on it. Documentation of training should be emailed to me.
  • Observed employee not wearing effective hair restraint while doing food prep.
  • Observed exposed wood under the mop sink. Throughout the kitchen there were utensils stored on top of paper towels. Paper towels and wood are not easily cleanable.
  • Underneath the soft serve machines there is an accumulation of buildup. I pointed this out to the owner.
  • Observed no self-closing device on restroom door in dining room.
  • Found no lid on waste can in restroom.
  • The cabinet below the icy machine had numerous mouse droppings in it. Suggested thoroughly cleaning cabinet while wearing a face mask and appropriate personal protective equipment. Per owner, there is a licensed pest operator that comes to establishment. He stated there was a mouse, but he put out a trap and caught it. He will clean the area and monitor for new droppings. The facility will be closed started Wednesday until next year. He will email after a week about whether there are new droppings and if there are what exactly he does (contact pest company)
  • Carpeted rugs are being used in the back storage room. Rugs are not easily cleanable. If slipping is an issue there are rubber mats you can put down.
  • Observed a piece of base coving missing along the wall to the right of the soft serve ice cream machine. Replace coving.
