- Observed heavily rusting shelves on chemical storage shelf and pots/pans shelf in main kitchen. Seal or replace shelves.
- Observed employee coats and purse being stored in cabinet on top of canned foods and near plates in West Commons. Ensure employee items are stored separately from facility's food and food surfaces.

- Observed paint chipping on yellow shelf that holds seasonings in kitchen. There is tape on the coffee stand. It appears that it's there to try to hold up a metal pieces. There is paint also coming off on the side of the same coffee stand. Observed broken seals in the white chest freezer in kitchen. Observed a crack in the top of the cart in kitchen baking area. Observed seal around hand washing sink in kitchen in poor repair in Commons West.
- The cups in the dining area have been stacked wet.
- There is buildup under the dish machine. Also in between cabinets in the dining area there is dust accumulation.
- Observed porous ceiling tiles in Commons East and West. Some of the tiles are stained. Replace with smooth easily cleanable tiles. In Commons West the wall appeared to be damaged by the hand sink.

- Observed paint chipping on yellow shelf that holds seasonings in kitchen and wooden island in kitchen. Observed broken seals in the reach in cooler in kitchen, white chest freezer in kitchen, and white freezer in Commons West. Observed cracked cart with debris build up in kitchen. Clean or repair. Observed seal around hand washing sink in kitchen in poor repair. Observed cabinet door damaged near two compartment sink in Commons East.
- Observed no self-closing devise on restroom door. Restroom door should be self closing.
- Observed gap around door to outside in both the cafe and commons east. Doors must be repaired so no daylight is visible as that means pests can enter. Discussed with dietary manager he should check all doors to the outside to make sure they don't have gaps either. Please repair the doors by 7-1-15. Contact Katie once the doors are repaired or if you need more time.
- Observed Christmas style ribbon used as coving in dining area. Replace with easily washable coving.
- Observed crackers and other debris under shelving units on floor in dry storage area in kitchen.
- Observed porous ceiling tiles in Commons East. Replace with smooth easily cleanable tiles. In Commons West the wall appeared to be damaged by the hand sink and another place.

- At the daycare there is a room with a beverage cooler to hold extra milks and snacks that has carpet.
- In the Commons West there is an ice machine that is propped up on blocks of woods that don't appear to be sealed. The wood is discolored from water damage.

- I observed minor damage on the seal of the small chest ice cream freezer in the main kitchen and also minor damage on the seal of the small chest ice cream freezer in the Common West kitchen.
- At the daycare there is a room with a beverage cooler to hold extra milks and snacks that has carpet. Discussed this with the Dietary Director.
- I observed black buildup under the 3-compartment sink that was adjacent from the deep fryer. Found build-up of debris on floors. Clean the floors. Found build-up of debris on floors. Clean the floors.
- I observed paint chipping on the wall below to hand sink in the Common West kitchen. Also there was slight damage to the wall below the sink in the produce wash area in the main kitchen.
- The ceiling tiles in the Common West kitchen were not easily cleanable. Discussed with dietary director possibly replacing tiles with ones that are easily cleanable.
- I observed what looked like food splatter on walls below the sink in the produce wash area.
