Observed employee using bare hands to put RTE sliced onions on burger, then put top bun on burger. Discussed with employee and manager about TCHD no Bare-Hand contact policy. Employee washed hands and put on new gloves. Ensure a barrier, either single use gloves, tongs, or deli tissue are being used to handle all ready to eat foods.
Observed employee cooking and preparing burgers without a hair net. Ensure all employees working with food have an effective hair restraint, such as a hair net or hat.

In the walk in cooler there was small plastic containers of sour cream and salsa date labeled 10-1. I explained once commercially prepared items are taken from their original container they must be date labeled and used within 7 days. They were voluntarily disposed of.
The sour cream and salsa was voluntarily disposed of.
There was cardboard on the floor under the fryer. Also there was cardboard by the 3-compartment sink that had chemicals on it. Cardboard is not easily cleanable and must be removed.
