Observed sliced ham in the reach in prep cooler labeled 1/15/16. The manager stated the Ham had been frozen and pulled at a later date, this was not documented. The manager voluntarily threw away the Ham.
Observed improper scoops being used in the bulk flour and sugar. Scoops should have handles that are extended out of the product.
Observed an employee use wet hands to turn the faucet off when washing hands. Discussed proper hand washing on site.
Observed an employee handling food without an effective hair restraint. A hat or a hair net should be worn when handling food.
Observed the hand sink faucet in the kitchen leaking. Repair.

Milk to consumers is served from gal jugs.

Found container of chili being thawed on top of table.
Cook was using bare hands to place sliced tomatoes, lettuce & potato chips on platter to serve to customers.
Found some single-use food containers being reused to store other food in.
Found two moist wiping clothes stored on counters.

Observed dust built up on the metal hanging shelf above the prep/ cold holding table in the middle of the kitchen. Please ensure shelf is clean to avoid possibility of dust falling into food.
