- Observed a container of ham labeled 3/2/16. Discussed potentially hazardous foods being used within 7 days. The food was voluntarily thrown away during the inspection. There was a package in the prep cooler without a label. Per an employee it was pulled pork. Discussed all food needing a name and date label. The food was voluntarily discarded during the inspection.
- Observed an employee handling food wash his hands and dry them with a hanging towel. I discussed how using a towel to dry hands may re contaminate them and using disposable paper towels will not. He provided paper towels to the hand sink.
- Observed an employee use wet hands to turn the faucet off when washing hands. I discussed how using wet hands to turn the faucet off can re contaminate hands.
- Provide an easily cleanable and non absorbing surface for the opening in the wall in the mop sink room. Observed shelves behind the bar with glasses stored on them, and shelves in the coolers behind the bar not sealed. Seal.Pizza boxes are stored on cardboard, remove the cardboard and seal the pallet that the boxes will be stored on. The shelves under the 2 fryers are being held up on one corner with a cardboard box. Items should be stored on surfaces that are non absorbent and easily cleanable. Remove or replace the cardboard. The counter top near the old to go window is broken and cracking. Repair or replace.
- Observed cups stored on a towel that is sticky behind the bar. Store cups on a non absorbent and easily cleanable surface.
- Observed the inside of the chest cooler containing soda kegs to have spilled soda build up. The cooler containing the sauces has food debris build up. The shelves in the kitchen under 2 fryers have debris build up. The handle to the cooler containing sauces has food debris build up. Clean and maintain routine cleaning of these areas and other non food contact surfaces.
- Observed the floor throughout the kitchen to have chipped paint. The floor under the 3 compartment sink is broken. Repaint or repair these areas.
- Observed the floor in the room with the to go window to have grease spilled. Clean and maintain routine cleaning. The floor under and around equipment, specifically under the prep table in front of the pizza oven, has food debris build up. Clean and maintain routine cleaning.
- Observed the wall partition in the kitchen by the prep area to be broken. Repair the wall so that it is easily cleanable and non absorbent. There are multiple missing ceiling tiles in the mop sink room. Provide ceiling tiles.
- The ceiling fan and the light fixtures in the pizza making area have dust accumulation. Clean and maintain routine cleaning.
- The ceiling vent in the to go window room has heavy debris build up.There are cobwebs in the corners of the room with to to go window and the 3 compartment sink room. The walls in the to go window room have grease build up. The walls in the room containing the 3 compartment has debris on the walls. Clean and maintain routine cleaning.

- Observed a bowl of ground sausage in the meat cooler with no name or date label. The person in charge voluntarily threw the sausage away. All potentially hazardous food should be labeled with a name and date.
- Observed shelves behind the bar with glasses stored on them not sealed. Seal. Counter top above can food storage is chipped. Repair.
- Observed the meat cooler and freezer portion of reach in cooler/freezer to have food debris build up. Clean and maintain routine cleaning.
- Observed the front door with gaps where daylight can be seen through it. Repair the door by 12/16/15. I will be back to ensure the door is repaired on that date.
- Observed the floor in the kitchen with debris build up around and under shelves. Clean and maintain a routine cleaning schedule.
- Observed the walls in the dish room area and frying area with debris build up. The ceiling tiles through out the kitchen have debris build up. Clean, and maintain routine cleaning.
- Observed extraneous items in the kitchen. Remove item unnecessary to the facilities operation.

- Found bare wood shelves inside two Refrs. Also found edges of several kitchen counters & 2X4 unsealed.
- Found food debris buildup on potato slicer.
- Found lent on fan & on some equipment, food containers, etc in kitchen.
- The womens restroom door does not closely properly.
- Found dumpster lid open.
- Part of the paint on the kitchen floor is bare.
- Found some small holes in kitchen. Also, found dry wall in kitchern by oven rough & unfinished.

- Observed damaged prep counter in the kitchen. Repair.
- Wash, rinse and sanitize the pizza oven rack and conventional oven; observed food debris build-up.
- Observed soiled door seal gaskets on reach in refrigerator. Was,rinse and sanitize.
- Observed fruit flies throughout facility and mice droppings on the white storage rack with stored soiled dishes. Facility must contact pest company to address these issues. A follow-up inspection will occur after 8/28/14 for compliance. Provide pest company invoice.
- Observed spider webs throughout kitchen and around 3-comp sink. Clean.
- No light shields provided for lights above fryer.
