- Observed employee washing dirty dishes remove clean and sanitized dishes without washing his hands. Hand shall be washing after handling dirty dishes before handling clean dishes. Inspector had employee stop and wash his hands.
- Inspector observed employee only rinse his hands for approximately 4 seconds after handling dirty dishes before handling clean dishes. Employee was retrained on site by manager and inspector on proper hand washing technique. Retrain all employees on proper hand washing and send verification to the Tazewell County Health Department, Attn: Kim by 1-25-15.
- The calk is peeling and black with debris along the wall juncture at the 3-compartment sink, the hand sink by the walk in cooler, and the hand sink by the chicken walk-in cooler. Replace damaged and soiled caulk.
- The fryers, Blodgett oven, and cooking equipment had a build up of debris. Wash, rinse, and sanitize them.
- The counters and shelves in the batter area had a build up of debris. Wash, rinse, and sanitize these items.
- The outside of the coolers and door handles throughout the facility had a build up of debris. The mop sink and hand sink outside of the chicken cooler had a build up of debris. Clean and sanitize them.
- Replace damaged and missing floor tiles under the fryers.
- The floors had a build up of grease and debris, especially under and around equipment and fryers. Thoroughly clean the floors.
- The walls had a build up of debris in the cook line, under and above the 3-compartment sink, inside chicken cooler, and behind hand sinks. Clean the walls.

- Observed 3 plastic warming pans that had a heavy debris accumulation on them. 1 of these pans were cracked and voluntarily discarded by the manager at the time of this inspection.
- Observed vent cover on fan in freezer that is coming off. Repair. Observed seal in warmer and in capri sun cooler that are coming detached. Seal. Observed seal on 3 compartment sink that is coming off. Reseal.
- Observed debris accumulation on the range cook top and the microwave. Clean and ensure routine cleaning.
- Observed debris accumulation on the following items surfaces: All metal racks in facility, soda machines, thermometer, rice container lid, biscuit freezer especially around the seals and the hot holding warmer. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed cracked and missing floor tiles in kitchen. Replace.
- Observed debris accumulation on floors throughout facility especially behind cooking equipment and in the dry storage area. Clean and ensure routine cleaning.
- Observed dust accumulation on ceiling in drive thru area. Clean and ensure routine cleaning.
- Chicken is being cooled down and reserved. Category 2 facilities must not cool down and reserve foods. Facility will have to obtain category 1 permit to be in compliance.

- The signs in the chicken breading station are coming off and are stained. The stand mixer has paint missing. The sealing behind the 3-compartment sink is peeling and has black buildup.
- In the back by the water heater there were 3 dead insects. Per manager she has not seen any pests in the facility. The facility does have a licensed pest company that treats the facility. The last time they were at the facility was 10-21-14. Discussed sweeping the area then monitoring to see if it is a cleaning issue rather than an ongoing pest issue. If you do see more insects make sure to notify your pest company.
- There's buildup on floors throughout the facility, but especially under equipment in the chicken breading and frying area.
- There are a couple of broken floor tiles under the fryer that is not being used.
