The mop sink in the kitchen edges were crumbling. There were gnats around this area, clean and maintain routine cleaning and repair/replace mop sink. Observed missing floor tiles in the kitchen throughout the facility. Replace/repair. Floor throughout the facility has debris build up. Clean and maintain routine cleaning. Observed grease build up towards the bottom of the wall behind the chicken frying area. Clean and maintain routine cleaning. Observed the coving of the wall crumbling on the corner of the wall next the the hot water dispenser. Repair/replace Not all employees have their food handlers permit. The Blodgett oven, hot holding units, fryers had a build up of grease and debris. Wash, rinse, and sanitize cooking equipment. Observes a build up of debris on the chicken prep cart and counters, the fryer basket cart, and the inside of the cabinet under the drive thru soda dispenser. Wash, rinse, and sanitize these areas. The outsides of Blodgett oven, hot holding units, fryers had a build up of grease and debris. Wash, rinse, and sanitize cooking equipment. Repair leaky faucet at mop sink. Found build-up of debris on floors in the cooking, prep, storage, and walk in coolers. Clean the floors. The floors on the cook line and prep areas and outside base coving by the chicken walk in coolerhad several busted tiles. Replace damaged floor tiles. The walls in the cooking and prep areas had a build up of debris. Clean the walls. New manager did not know where food handler training certificates were located. She did not think anyone has it.
