- The door seal to the small under-counter freezer at the bar was broken. Repair or replace it so it is smooth, durable, non-absorbent, easily cleanable and in good repair.
- The pizza ovens had a build up of grease/debris. Clean and sanitize them.
- The outsides of the kitchen cabinets had a build up of grease/debris. Clean and sanitize them.
- There was a hole in the banquet furnace room wall/ceiling which was allowing birds to enter the room. Repair the hole by 7/2/15 to exclude pests.
- Found 3 pieces of mouse feces in a drawer in the bar area. The drawer was immediately cleaned and sanitized by the bartender. Provide a copy of the pest control receipts to TCHD, Attn: Kim by 7/2/15. There were 2 birds inside the furnace room in the banquet room and bird feces on the floor. The birds must be removed and the room cleaned and sanitized by 7-2-15.
- Found build-up of debris on floors under and around equipment. Clean the floors.
- There were holes in the wall right inside the storage room off the entryway, outside the walk in cooler door, in the storage room behind the kitchen and women's auxillary storage room. Repair the walls. Cover exposed insulation. There was missing trim next to the banquet room doorway. Replace the missing trim. There was an unfinished area on the ceiling in the banquet room and bare wood behind the ice machine on the walls and in the storage rooms. Paint or seal these walls. There was a damaged ceiling tile behind the northeast T.V. in the bar area. Replace it.
- There was a build up of debris on the walls and ceilings in the walk in cooler. Clean and sanitize these areas.
- Found an unlabeled spray bottle with the cleaning chemicals. Chemicals must be labeled to avoid miss use. It was emptied out.

- Found sour cream date labeled 3/13 in the white fridge in the kitchen. It must be labeled and used within 7 days. Found unlabeled baked beans in the white fridge in the kitchen. It must be date labeled and used within 7 days. These foods were discarded by the manager.
- Found a cardboard box with salad mix and dressings in it on the floor in the walk in cooler. Store food at least 6 inches off the floor.
- Found a rubber spatula in the utensil drawer in the kitchen the was broken and missing pieces. Replace it.
- Observed damaged door seal gaskets to the small white under counter freezer at the bar.
- The microwave, shelf under fryers, pizza ovens had a build up of grease and debris. Wash, rinse, and sanitize these areas.
- The kitchen cabinets and dishes in these cabinets , counters, bar trough cooler on the west end of the bar, counter by the pizza ovens in the bar area, shelves in the kitchen and bar areas had a larger build up of debris and mouse feces (30+droppings i n each area). The fridge/freezer in the kitchen had a build up of debris inside it. Clean and sanitize it.
- The outsides of the kitchen cabinets, coolers, top of the grease trap, kitchen cart with roasters stored on it. Clean and sanitize these areas.
- Found a box of single service
- Observed 30+ mouse droppings in each of the following areas: Inside almost every kitchen cabinet and drawer, in the spice cabinet, on the counter in the bar area by the pizza ovens, on the floors in the kitchen and bar areas, on the floor in the large room south of the kitchen, on the shelves in the furnace room south of the kitchen, in the west bar trough ice cooler. Clean up all mouse feces and disinfect these areas. Also, found a live mouse stuck to a glue trap behind the bar. Employee stated that Orkin has been handling the mouse issue. Provide Pest Control receipts to TCHD by 5/20/15.
- Found several gaps and holes coming into the facility from the outside. Seal these gaps and holes to prevent pests entering the facility.
- The were some damaged floor tiles and a raised floor in the bar area with gaps under it. Repair the floors and replace cracked tiles.
- Found build-up of debris on floors. Clean the floors. Remove all mouse droppings throughout the facility.
- There was a very large hole in the wall behind the ice machine (missing cinder block) and a hole in the wall at the front of the outside of the walk in cooler. Repair the holes in the walls.
- The walk in cooler walls and ceilings had a build up of debris. Clean and sanitize them. The kitchen walls had a build up of debris. Clean the walls.
- Found a spray bottle of Crew bathroom cleaner stored with mustard packets and over salt shakers on a shelf in the kitchen under the pass through window. Employee moved this to chemical storage area.
- Found an unlabeled spray bottle of clear liquid in the bar area. Corrected--Employee emptied this bottle immediately.
- Found a dirty mop head stored on the floor of the walk in cooler.
- Found cooked and cooled taco meat and pulled pork in the kitchen freezer/fridge the was unlabeled. This facility is a Category 2 facility and cannot do cool down. Discontinue cooling down foods immediately and contact TCHD if you wish to upgrade to a category ! facility which is allowed to cool down foods. The above foods must be discarded.
