- No thermometer was available at the facility to be calibrated at time of inspection. Manager purchased a thermometer during the inspection. A thermometer calibration handout was given.
- Observed a plate of food and boxes of food on the floor in the back walk in freezer. There were bread sticks on the ground in the back storage area. Per the manger, the bread sticks are meant to be thrown away and an employee will throw them away. Food should be stored at least 6 inches off the ground at all times.
- Observed an employee eating in the kitchen by the ovens. Employees should only eat in designated areas. Spoke with the manager about employees eating in the kitchen.
- Observed plates being stored on cardboard near the entrance to the kitchen. There are boards that are white being used to line shelves on the opposite side of the kitchen line. Only non-absorbent and easily cleanable surfaces should be used. The shelving unit along the wall holding plates is chipping. Repair/repaint.
- Observed the 3 compartment sink with only a wash and a sanitize made up. During the inspection the 3 compartment sink was made up with wash, rinse, and sanitize. I will send a 3 compartment sink handout with the inspection.
- The hand sink at the end of the 3 compartment sink is broken and not in use. Repair the hand sink. The hand sink by the utensil storage is leaking from the back. Repair.
- Observed the following areas without proper hand washing items: the hand sink by the walk in cooler without paper towels or hand soap, the hand sink by silverware storage without paper towels, the hand sink in the Pepsi dispenser area without paper towels or soap, and the hand sink behind the bar without hand soap. Per the manager, an employee is bringing paper towels and soap for all hand washing sinks.
- Observed a gap at the bottom of the door in the kitchen that exits towards the garbage bin. Discussed using weather stripping on the bottom of the door to close the gap. The gap should be fixed so that day light cannot be seen by 2/4/16.
- Observed dust build up on walls of kitchen near utensil storage. Clean and maintain routine cleaning.
- The facility has no verification of food handlers of food managers in the facility. I suggest keeping a document containing; employees, their date of hire, and when they completed their food handler training.
- The facility was not able to provide cool down documentation or temperature logs. Per the manager the chef took them home. Provide me 2 weeks of cool down documentation and temperature logs by 2/8/16. I will send temperature documentation handout with example documentation sheets with the inspection.

- Found uncovered food in the coolers and freezers throughout kitchen. Cover foods in storage.
- Repair holes in the doors going into the kitchen from the dining room. Repair the broken door seal to the walk in freezer.
- The shelves in the dry storage area, can opener blade, shelves in the coffee area in the kitchen, and insides of coolers throughout the kitchen had a build up of debris. Clean and sanitize these areas.
- The outsides of the coolers had a build of debris. Wash, rinse, and sanitize them.
- Repair the leak from the pipe at the top of the dishwasher and the drain under the hand sink by the dry storage area in the kitchen.
- The dumpster lid was open. Keep the dumpster lid closed. Corrected--it was closed.
- Observed drain flies in the storage area in the kitchen. Clean the drains and eliminate these flies.
- The back door of the facility by the dumpster has a gap to the outside under it on one end. Replace the missing piece of the door sweep.
- Paint or seal the bare concrete floor in front of the walk in cooler and freezer and in the dry storage areas in the kitchen.
