- Observed unlabeled spices in white, bulk containers. Containers were properly labeled by employee, on site.
- Observed employees without effective hair restraints. All employees shall wear hair restraints while working with or around food.
- Observed debris buildup in the following areas; on stove top, on bottom shelf in white stand alone freezer, and on utensils storage shelf. Ensure routine cleaning on all non-food contact surfaces.
- Observed bathroom doors that were not self closing. Discussed with manager that documentation must be submitted to health department as to why self closing latch is not present.
- Observed kitchen ceiling tile replaced with cardboard. Remove cardboard from ceiling and replace with non-absorbent, durable, and easily cleanable ceiling tile.
- Ceiling tile above pizza dough prep area is damaged. Repair or replace ceiling tile.
- Observed dust and debris on walls and around window near spices storage. Also observed debris on front of walk-in freezer door. Ensure routine cleaning.
- Observed dust buildup on ventilation ducts above stove top area and above cooler in main service station.
- Observed sanitizer spray bottle stored near food and food contact surfaces. Manager removed spray bottle on site. Ensure all chemical storage is in a separate area from food and food contact surfaces.
- Observed sanitizer solution being stored in a previous chemical spray bottle. Ensure all chemical solutions are stored in their original containers. Spray bottle was discarded by manager during inspection.
- Not all staff in the facility are in compliance with food handlers training. Ensure all staff are certified with food handlers training within 30 days of hiring.

- Observed a bottle of sanitizer spray stored on a shelf above food utensils. Chemicals must be stored physically separate and below food or food storage containers.Bottle was removed by staff at time of inspection.
