- Repaint the rusty metal shelves in the walk in cooler.
- Caulk between the wall and the back of the 3-compartment sink and hand sink in the kitchen so these areas are easily cleanable. Remove the tape and repair the door seals on the kitchen prep cooler and all three chest freezers. Tape is not a smooth, durable, non-absorbent, easily cleanable surface.
- The following areas had a build up of debris: pipes behind the fryers, door seals on the prep cooler in the kitchen, and the outside of the lettuce container in the walk in cooler. Wash, rinse, and sanitize these areas.
- Repair/replace the damaged floor areas outside the walk in cooler and in the room with chest freezer #1 and #2.
- Replace missing base coving under the 3-compartment sink in the kitchen and under the storage shelf on the north wall in the kitchen. Repair the large hole in the wall in the room with freezer #3.

- Observed half gallon of milk in prep-counter cooler that was not labeled. Ensure milk is labeled after opening and used or consumed within 7 days.
- Observed two thermometers that read 40 F in ice bath. These thermometers should read 32 F in ice bath. Calibrate thermometers or replace and ensure routine calibration.
- Observed tape on the deep freezer labeled #2. Remove tape as it is not an easily cleanable surface. Observed rust accumulation on racks in walk-in cooler. Replace or reseal. Observed rust accumulation on freezer 2. Replace or repaint. Observed broken seal on freezer 1. Replace.
- Observed debris accumulation on the following items/surfaces: Ledge in ice machine, soda machine, outer plastic container in walk-in cooler where lettuce is stored. Clean and ensure routine cleaning
- Observed packages of to-go cup lids on the floor in the bar. Single service items must be a minimum of 6 inches off of the floor.
- Observed a pitcher stored in the hand sink in the bar. Per the owner, the pitcher was drying in the handsink. Handsinks are to be used for hand washing only. The pitcher was removed at the time of this inspection.
- Observed missing/chipped floor tiles in the kitchen in front of the walk in cooler and in the soda/freezer storage room. Repair.
- The coving beneath the 3 compartment sink has come off. Repair or replace.
- Observed a hole in the wall where freezer 3 is stored. Repair.
- Observed two bottles of chemicals that were unlabeled. Per the owner, these bottles contained windex and a multi purpose cleaner. These bottles were labeled at the time of this inspection.

- Observed two cartons of eggs in walk in cooler that has a use by date of 4/18/15 and 4/28/15. Both cartons of eggs were voluntarily discarded by employee at time of inspection. Observed 6 ramekins of salsa in cooler dated 4/3/15. These 6 ramekins of salsa were voluntarily discarded by employee at time of inspection.
- Observed employee doing food prep without the use of a hair restraint.

Observed rust accumulation on freezer. Replace or repaint. (Per manager, she has a new freezer which she plans on replacing the rusty freezer with).
- Observed build up on ledge in ice machine. Clean and ensure routine cleaning,.
- Observed damaged/missing floor tiles in kitchen and storage room. Per manager, the floor tiles will be replaced this summer.
- Employee does not meet food handler requirements. Ensure all staff has met the food handler certification requirements by 6/7/15.
