- Observed unlabeled hash in a small container in true cooler in the kitchen. The item was not date marked. Staff voluntarily discarded food item during inspection.
- Observed commercially produced green beans being heated in a hit holding unit in the kitchen. kitchen staff said they had put it in green beans within half an hour. Temperature of beans was at 83 F. Inspector advised staff to remove green beans and heat to a minimum of 135 F before hot holding.
- Did not observe use of gloves by staff during food preparation. Use gloves.
- Observed gasket of white cooler was broken and duct taped from top top to bottom. Remove duct tape and replace gasket.
- Observed handles of white chest freezers in the back storage area are broken and there are holes where the handles used to be. Replace handle. Observed surface of the inside of the cabinet under coffee machines (in the front area) are not sealed. Seal or paint. The plywood in the second of same cabinet is in a state of disrepair. Repair. Top of wood structure supporting dish washing machine is rotting away. Replace and seal surface or use material that is not prone to water damage.
- Observed debris in the utensil drawers below the toaster in the kitchen. Clean. Observed debris build up in the following areas: gaskets of the cold holding/prep unit; inside the True cooler, especially the shelves, corners, and gasket; top and bottom part of machine especially in corners; inside True freezer, especially the lower part and the gaskets; inside of the soda machine nozzles. Clean.
- Observed debris build up on the outside of the following equipment: door of the cold holding unit, including the handles; outside of the true cooler including the frame; and the outside of the microwave. Clean.
- Observed duct tape on floor in the back storage area by back door. Remove and repair floor. Observed broken tiles in storage room by kitchen entrance and bathroom in kitchen. Repair or replace.
- Observed use of plastic to plug hole of wall/floor juncture, coven was also missing. Remove plastic and repair. Ensure coven is installed. Observed base coven missing in on the wall by the counter in the front room, and the second dining area, on the wall by the ladies bathroom. Install base coven.
- Observed some parts of wall in the back storage area (close to the floor) and entrance to the back storage area peeling away. Repair.

- Observed employee cooking with hair only pulled back in a ponytail. Always wear a hat or hair net when working in the kitchen.
- The counter on the window between the wait station and the kitchen is bowing up. Repair the counter. Observed chipping wood on the shelves in dry storage. Repaint the shelves. Observed cardboard under the microwave. Cardboard is not non-absorbent and easily cleanable. Remove the cardboard.
- Observed black build up on the corners of the ice machine drip tray. Wash, rinse, and sanitize on a routine basis throughout the day.
- Observed food debris in the microwave. Wash, rinse, and sanitize the inside of the microwave on a routine basis throughout the day.
- All non-food contact surfaces must be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the bottoms of reach ins and the sides of equipment.
- Observed broken floor tiles in dry storage. Repair or replace tiles.
- Observed bowing and cracked ceiling tiles. Replace tiles.

- Observed employees not wearing effective hair restraints. Always wear a hair restraint in the kitchen/prep area.
- Observed build up on the ice machine drip tray. Wash, rinse, and sanitize drip tray.
- Observed wrenches being used as faucet handles for 3-compartment sink. These are not proper handles. Replace with faucet handles.
- Observed stained ceiling tiled throughout kitchen/prep/storage area. Replace ceiling tiles.
- The sealant on the handsinks in the kitchen and men's room is severely cracked. Reseal handsinks to the wall.
- Mop was in the bottom of the bucket. Store mop in the grated portion of the mop bucket or free hanging so it will air dry.
