Observed various foods in 3-door reach-in fridge with internal temperatures above 41 degrees
Food has been in food overnight (over 16 hours)
Food has been discarded intro trash by employee (see Voluntary Condemnation & Destruction Form)
Keep all foods held in cold temperature at 41 degrees or lower
Observed live roaches on floor and wall under hot holding units
Observed dead roaches in various areas
Clean and sanitize areas under hot holding and where dead roaches are
Continue using professional pest control
Fax pest control invoices.

Small refrigeration unit at 47 degrees employee stated that she checked unit this morning and it was at 38 degrees facility is checking temperatures often
Please start writing down temperatures so we can monitor the refrigerator unit
Must hold temperatures at 41 degrees
Hand wash station is going to be installed.

Facility currently does not have a hand wash sink currently using a 2-compartment sink
Small refrigerator unit at 56 degrees - call serviceman, must be at 36-38 to ensure food temperature is at 41 degrees or less
Removed items that have been stored less than four hours and transferred to another unit
The items more than four hours (potato salad, salsa) were discarded.
