Observed no hand soap at handwash station
Employee states they are using dish soap to wash hands, stock soap and paper towel dispensers before preparing food to assure hands are washed properly
Reach in unit holding food not working properly
Food brought out from walk-in holding at 41 degrees after 8 a.m
Check temperatures at 10 a.m
Containers set on ice
Only small for lunch time and rush hour food must be held in walk-in the remainder of the time
Provide sanitizing solution of 100ppm to sanitize contact surfaces
Corrected on site
Provide consumer advisory for \"ceviche\" cooked with lemon juice not under heat
Observed many flies in facility
Observed doors open
Keep doors closed
If problem continues obtain approve device to eliminate fly activity.
