- Rice and flour are considered to be adulterated and can no longer be used as food/food ingredients to be served to public

- Requesting owner to have commercial repair service to evaluate/trouble shoot electronic thermostat system
- All parts of the system must maintain the food at 135 degrees or higher
- Requiring all doors to be self-closed to eliminate pest
- Door on Hwy 65 side does not close all the way
- Requesting door be serviced/adjusted
- Requesting removal of items under the wood storage shelf that are no longer used
- Everything must be 6 inches off the deck
- All remaining violations have been corrected.

- Gloves need to be removed after handling cash
- Refrigerator in kitchen does not have internal thermometer and is out of temperature
- Refrigerator either needs to be replaced or fixed
- Observed no sanitizing bucket and no test strips
- Need to have a sanitizing bucket with chlorine reading at 100 ppm available at preparation area
- Also, need to provide test strips
- Observed flies in entire facility
- Recommend putting in a fly fan at entrance.
