- All but following still need correction: Hire professional pest control company to spray for cockroaches and any other found vermin
- Fax copy of pest control service receipt
- 3-part sink was replaced with newer sink and is clean
- Sink is approved for warewashing and handwashing
- Inspector provided chlorine test strips and demonstrated how to use them for wiping cloth and warewashing solution (dissinfecting)
- Operator stated this machine is down now but is used during summer months
- All soda nozzles must be removed and dissinfected every 1-2 days when in use
- Good job in removing all trash from floors and counters
- Food prep sink (single compartment sink in kitchen) is to be used to wash the chicken (food only) and still needs some cleaning, especially around bottom edges inside basin.

- Employees are not practicing basic food safety procedures, such as proper handwashing, utensil/equipment sanitization, and proper use of wiping cloths
- All food facility employees must be retrained by person who is food safety certified
- No handwash cleanser at any handwashing facility, including restroom
- Handwash compartment of 3-part sink and restroom sink must be provided with both single-use paper towels and handwashing cleanser in their own dispensers (no dispensers present either)
- Observed grease encrusted all over thermometer (probe) in food warmer
- Dial reading could not be taken
- Clean and sanitize (using bleach/ammonia solution) this thermometer and maintain it clean
- Food contact utensils/equipment shall be cleaned and sanitized regularly to prevent substance accumulation
- Observed live and dead roaches throughout
- Employees stated that facility does not have regular professional pest control service
- Problem will continue if facility is not cleaned
- Facility shall be kept free of vermin, rodents (rats, mice), cockroaches, flies
- Hire professional pest control service immediately and provide proof of this service to Health Officer
