- 3020 - Soap was not provided at the hand washing lavatory at the bar.
Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands. - 3350 - Unnecessary poisonous or toxic materials found in the food establishment.
Remove unnecessary poisonous or toxic materials RAID from the premisis.

- 0820 A 2 - Critical Repeat Foods in walkin are above 41 degrees allowing for microbial growth
Take necessary steps to lower food temperatures under 41 degrees. - 3660 - Repeat Operating the facility without a permit
ABC may be contacted if facility does not renew permit by 4-13-09
Documents on safe food handling written in Korean provided to owner. Facility must renew permit, permit renewal given to owner. Notice of Violation issued on 4-10-09 regarding above violations.

Acquire and use a food temperature monitoring device.
Comments:
Beef in walkin cooled by storing in tubs of ice. Monitor food temperatures to ensure cold hold foods remain below 41 degrees.

Take steps to immediately lower food temperatures to under 41 degrees - 3660 - Facility operating with an expired permit.
Permit renewal application given to person in charge
Consumer advisory present on menu, need asterik to match menu items with advisory. Letter from supplier stating sushi is frozen for correct time and temperatures to be faxed to the Health Department. Person in charge stated all meats cooked to 165 or higher.

Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted.
