The operator provided a digital metal probe thermometer, chlorine and quat. test strips and a posted Employee Health policy during today's inspection. Management has instituted a policy where the managers that are in the food processing areas are to wear hair restraints
  • Cloths - Wiping Cloths - Use Limitation (corrected on site) (repeated violation)
    Observation: Wiping cloths improperly stored between use sanitizing solution test at 0 ppm chlorine when tested).
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Plumbing System Maintained in Good Repair
    Observation: Leaks were observed from the exterior welded areas of the rinse and sanitizing sinks, of the 3 compartment warewashing sink.
    Correction: Plumbing systems and components shall be maintained in good repair.

The operator has corrected the previous violations noted with the repairs pending to the rear storage ceiling area. At this time, I noted that the drive-thru windows were closed when there were no customers present at these windows.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: An open space and a leak was observed from the ceiling tiles, in the rear storage area.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

The operator provided a metal probe thermometer, chlorine and quaternary ammonium test strips and an Employee Health policy during today's inspection.
  • Cloths - Wiping Cloths - Use Limitation (corrected on site)
    Observation: Wiping cloths improperly stored between use ( the cookline and front service sanitizing concentration measured 0 ppm chlorine and wet wiping cloths were observed on the cookline).
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: The burrito egg mixture in the side prep area 4-door reach-in cooler was cold holding at improper temperatures.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Temperature Measuring Devices (repeated violation)
    Observation: There was no temperature measuring device located in the side upper French fries freezer cabinet and small cookline Beverage Air reach-in freezer.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents ( both drive-thru windows were observed open, even when there were no customers at the windows).
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities in Good Repair
    Observation: Hanging metal ceiling tile support structure and a leak was observed in the rear storage area.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Critical: Pests-Controlling Pests*
    Observation: Observed methods are not being used to control pests ( live flies were observed flying and landing on food and non-food contact surfaces in the rear prep/cookline and front service and dining areas)
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.
  • Pests - Removing Dead or Trapped Birds - Insects, Rodents, and other Pests
    Observation: Dead or trapped flies were observed heavily littered on the glue board, of the rear prep/drive-thru area pest control device
    Correction: Remove dead or trapped birds, insects, rodents, and other pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
  • Critical: Toxics - Storage of Toxic Containers* (corrected on site)
    Observation: A degreaser spray bottle was observed stored adjacent to a spray bottle of cooking spray by the cookline bread buttering machine. Also noted were boxes of a food flavoring stored between boxes of hand sanitizers and a bag of ice melt, in the rear storage area.
    Correction: Containers of cleaners must be located in an area that is not above food, equipment, utensils, linens or single service items.

  • Equipment - Good Repair and Proper Adjustment
    Observation: Ponded water was observed on the floor underneath the rear racked bag-in-a-box system
    Correction: the bag-in-a-box soda dispensing system was observed in a state of disrepair.
  • Light Bulbs Protective Shielding
    Observation: Light bulb in the ceiling light fixture before the ice machine was observed not shielded, coated, or otherwise shatter-resistant.
    Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.

At 6:07 AM 01/05/2021 the young lady at the pick up window had her hand inside of my tea cup as she put it under the tea container spigot. She did not even have on gloves. Then I saw her do it for the order behind me. It is crazy that with the times we are going through people are still doing that kind of thing. If employees do that sort of thing in front of you, it makes me wonder what they do when we ate not looking.
