• 0830 - Critical The prepared ready-to-eat (RTE) tomatoes and other items in the refrigeration unit is not properly dated for disposition.
  Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
• 1150 - The nonfood contact surface of the basement reach in frig has cardboard accumulating mold and is not designed or constructed to be easily cleanable.
  Alter or replace this item to provide a surface that is free of unnecessary ledges, projections, and crevices.
• 1800 - The nonfood contact surface behind the ice machine, grill, walk-in freezer, as well as the vents in the kitchen and dining area has accumulations of grime and debris.
  Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
• 2720 - Observed filled refuse containers stored in the establishment containing food residue wastes without lids. The recepticle in the women's restroom also needs it's cover replaced.
  Cover all waste containers when not in continuous use.

• 0820 A 2 - Critical Cold holding at improper temperatures. Tuna and cottage cheese at 44F.
  Cold hold potentially hazardous food at 41F or below to inhibit the growth of harmful bacteria. Check unit and repair so that it is capable of maintaining phfs at 41F or below. Also make sure foods are below 41F before placing on unit and do not overfill containers.
• 1770 C - Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: doorways of reach in unit and floor fan.
  Maintain nonfood-contact surfaces of equipment clean.
• 3170 - Ceiling in grill area is not maintained in good repair-paint peeling.
  Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
• 3380 - Corrected During Inspection Critical Chlorine being applied to food contact surfaces does not meet the requirements of 21 CFR 178.1010. Wiping cloth concentration below 50 ppm.
  Utilize only chlorine that meet the requirements of 21 CFR 178.1010 when applying to food contact surfaces. Keep concentration at 50-100 ppm-1 tbsp bleach to 1 gallon of water.

• 1770 C - Repeat Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: front of soda dispenser.
  Maintain nonfood-contact surfaces of equipment clean.
• 3180 - Repeat Floor vent and ceiling vents noted in need of cleaning.
  All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
• 3200 - Vent filters in the hood system are not being maintained in a clean condition. Some grease accumulation in filters.
  Maintain hood system vent filters in a clean condition.

• 1770 B - Interior top of ice machine has small amount of build up.
  Clean regularly.
• 3045 - Corrected During Inspection No handwashing signs in restrooms.
  Post handwashing signs at all sinks where employees wash hands.
• 3180 - Floor under grill, floor vents and ceiling vents dirty.
  Clean as needed.

February 15, 2005 (Routine)

• 0760 - Corrected During Inspection Critical Chili beans in warmer at 130 degrees F.
  Reheat previously cooked foods to 165 degrees F. within 2 hours than hold over 140 degrees F.
• 1770 C - Front of soda fountain dusty.
  Clean as needed.
• 3170 - Repeat Pipe leaking under stairwell downstairs. Floor tile missing around 3 compartment sink.
  Repair leak and replace tile.
• 3180 - Fan and wall behind ice machine dusty.
  Clean as needed.

• 3180 - Conduit, wall behind fryer dirty. Ceiling vent and wall by grill and light fixture dusty.
  Clean as needed.
• 2890 - Lights in grill area not shielded.
  Shield all lights properly.
