  Replace the hinge on the lid with an approved and cleanable product.
• 3170 - The walls, ceining tile and florr tiles have reached a point of the need for repair/replacement.
  Submit a plan for upgrade of the physical facility before issuance of next annual permit.

March 23, 2009 (Routine)

• 0820 A 2 - Critical Product on top of Pizza Prep Table cold holding at improper temperatures.
  Cold hold potentially hazardous food at 41F or below to inhibit the growth of harmful bacteria.
• 1900 - Critical When tested, no chemical sanitizer was found in the wiping cloth spray bottle used on food contact surfaces..
  Provide Quaternary Ammonia at proper concentration of 200 ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.
• 1900 - Critical Repeat Due to improper operation of the 3-Compartment Sink, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 12 VAC 5-421-1700)
  Verify that the 3-Compartment Sink is properly setup to Wash/Rinse/Sanitize and allow equipment/utensils to air dry .
• 2810 - Floor or floor covering in kitchen is not smooth and easily cleanable (missing floor tile).
  Repair or replace floor or floor covering to make it smooth and easily cleanable.
• 2930 - Outer opening of the food establishment is not protected against entry of insects and rodents (torn screen door).
  Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

• 0470 A 4 - Observed the following food was observed stored without being in packages, in covered containers, or wrapped: product stored on the top of Pizza Prep Table.
  Store food in packages, covered containers, or wrappings.
• 0820 A 2 - Critical Pepperoni, ham, and shredded cheese observed cold holding at improper temperatures.
  Cold hold potentially hazardous food at 41F or below to inhibit the growth of harmful bacteria.
• 2920 - Toilet room door is not provided with a self-closing door
  Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
• 3170 - Ceiling and floor tiles observed not maintained in good repair
  Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
• 3200 - Intake and exhaust air ducts / wall mounted fan are not being cleaned
  Clean intake and exhaust air ducts and change filters so they are not a source of contamination by dust, dirt and other materials

• 2350 ii - Plumbing connections at the 3-Compartment Sink observed leaking.
  Plumbing systems and components shall be maintained in good repair.
• 2890 - Light bulb in hallway outside walk-in cooler not properly shielded, coated, or otherwise shatter-resistant.
  Shield or replace light bulb with a coated or shatter-resistant bulb.
• 3220 - Repeat Mops not hung up to air dry.
  Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
• 3330 - Critical Working container of sanitizer in spray bottle are not properly labeled.
  Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.

February 06, 2007 (Routine)

• 1320 - The temperature measuring device in the walk-in cooler was not properly located in the warmest part of the unit.
  Relocate the temperature measuring device to the warmest part of the refrigeration unit to properly represent the ambient air temperature.
• 1320 - There was no temperature measuring device located in the GE refrigerator.
  Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
• 1530 - There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
  Obtain the proper sanitizer test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
• 3220 - Mops not hung up to air dry.
  Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
