    Observation: In-use utensils improperly stored between use.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.
  • Cloths - Wiping Cloths - Use Limitation
    Observation: Wiping cloths improperly stored between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Refried beans on the food prep. steam table and chicken legs and fajita mix on the hot bar are hot holding at improper temperatures.
    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.

    Observation: The prepared ready-to-eat (RTE) cooked chicken, cooked ground beef, salsa, and refried beans in the walk-in refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Equipment and Utensils - Multiuse, Characteristics
    Observation: The food contact surface of the lava rock bowls are not durable, nonabsorbent, easily cleanable, resistant to pitting.
    Correction: Replace the lava rock bowls with a different serving platter that will permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: commercial can opener, prep. tables, and cutting boards.
    Correction: Clean and sanitize these surfaces for food contact.

    Observation: The nonfood contact surface of the refrigeration units has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Outer opening of the food establishment in the kitchen is not protected against entry of insects and rodents.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Soap was not provided at the hand washing lavatory at the server's station.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Floors and walls in the walk-in cooler and throughout the kitchen/food prep area are in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
  • Mops - Drying Mops
    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

The PIC has corrected all violations. The walk-in cooler has been reorganized to provide separation of raw animal foods from RTE foods as well as raw vegetables. The PIC has also placed placards in the food prep area to help train other food handlers about the correct hot and cold holding temps as well as cooking temperatures. The cold holding salad bar table has also been fixed and food is being cold held around 35'F at the time of this inspection. Menus have also been amended to reflect a consumer advisory for raw or undercooked foods prepared to a customers request.

    Observation: The person in charge failed to explain the significance of the relationship between the holding time and temperature of potentially hazardous food and foodborne illness.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.

    Observation: The person in charge failed to state the required temperatures and holding times for the following conditions of potentially hazardous food: (1) Refrigerated storage, (2) Hot holding, (3) Cooling, and (4) Reheating.

    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration units. Observed Raw beef mixture stored above lettuce in the walk-in cooler and raw shrimp stored above baked potatoes in the Sand. Prep. Cooler.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Cooked Mexican Rice and Cooked Ground Beef hot holding at improper temperatures.

Sneeze guard needed at the future salsa bar. Relocated raw shrimp to prevent drip into other foods. Went over healthy food handler info with PIC. Covered damaged cans and food storage requirements with PIC. RIC at bar only contains beer. Ticket display to be replaced with an approved ticket holder, cable and clothes pins were removed.
