    Observation: The screen on the (back entrance door) screen door is torn.
    Correction: The screen needs to be replaced, in order to prevent entry by flying insects.

  • Equipment - Good Repair and Proper Adjustment
    Observation: There are rust and holes in the steel shelf, below, and associated with, the steel table on the "dirty dishes" side of the mechanical dishwasher.
    Correction: All shelves and tables must be smooth and easily cleanable, at all times. This steel shelf is "worn out" and is no longer cleanable at the bacterial level. This shelf needs to be repaired or replaced.

  • Critical: Equipment and Utensils - Before Use After Cleaning*
    Observation: The mechanical dishwasher was getting no detectable sanitizer, associated with the sanitizing step (the final rinse step) of the mechanical dishwasher cycle.
    Correction: We require that the available chlorine level be 50 to100 ppm on the sanitizing step (the final rinse step) of each run of the mechanical dishwasher cycle, as measured by the bleach-type test strips. Until the mechanical dishwasher is again sanitizing properly, all items must be sanitized manually, by immersion for at least one minute in a sanitizing solution, using the three compartment sink, following the run through the mechanical dishwasher. A service technician , on-site, was immediately contacted by the foodservice management, to work on the problem. We require that the foodservice management call us, and leave a message, once the problem has been corrected, and the mechanical dishwasher is again sanitizing properly.

    Observation: The mechanical dishwasher was getting no detectable sanitizer associated with the sanitizing step (the final step) of the mechanical dishwasher cycle, as measured by the appropriate test strips.
    Correction: The available chlorine level, associated with the final rinse step, must be 50 to 100 ppm on every run of the mechanical dishwasher cycle, as measured by the bleach type test strips. Until the mechanical dishwasher is again sanitizing properly, all items must be sanitized manually , using the three compartment sink, following the run through the mechanical dishwasher. Immersion in sanitizing solution needs to be at least one minute, before air drying. A service technician on-site was immediately called and began working on the problem. We require that we be contacted by phone, by the foodservice management, once the problem has been corrected, and the mechanical dishwasher is again sanitizing properly.

  • Critical: Hands - Preventing Contamination from Hands* (corrected on site)
    Observation: A cook was observed contacting celery with bare hands when cutting up the celery.
    Correction: We do not allow bare hand contact with "ready-to-eat" foods. Disposable gloves and tongs and other utensils (with handles) need to be used. This is true even when the "ready-to-eat" food may be cooked or warmed, prior to service to customers. This violation was immediately corrected by the establishment management.
  • Nonfood Contact Surfaces (corrected on site)
    Observation: Milk crates are being used as a substitute for smooth, easy to clean proper shelving, in the walk-in refrigerator.
    Correction: Milk crates have rough surfaces.They are not cleanable at the bacterial level. Replace using proper shelving or other smooth, easily cleanable, moisture resistant materials. If wood is used to build shelving, the wood must be sealed or painted.

  • Kitchenware and Tableware (corrected on site)
    Observation: Single-use disposable plastic knives (not individually wrapped) were facing in both orientations , in the storage box, in the customer self-serve area, next to the trays.
    Correction: All of these knives need to be facing in the same direction, in the storage box. This way, when one reaches for a knife, one will not touch the "eating" surface (the blade) of another knife in the storage box. This violation was immediately corrected by the establishment management. We recommend, but do not require, that individually wrapped plastic ware be used.

  • Critical: Package Integrity* (corrected on site) (repeated violation)
    Observation: There was a can of black olives slices, and there was a can of apple sauce, which had dents on top seams, but were being stored among canned goods which are intended for service to customers.
    Correction: Canned goods with dents in top, bottom, or side seams may not be used for service to customers. Cans with dents in seams need to be segregated from cans intended for service to customers, as soon as deliveries arrive, and cans are unpacked. Cans with dents in seams may be stored in an area specially designated with a sign (which may be hand-written) which says "damaged cans only," until the cans with dents in seams may be returned for reimbursement. Or they may be discarded. This violation was immediately corrected by the establishment management.
  • Backflow Prevention Device, Design Standard
    Observation: One of the two faucets in the mopsink/service sink is missing the required "back-flow prevention hose bib.
    Correction: Both the hot water, and the cold water, faucets in the mopsink/service sink area, are required to have a "back-flow prevention hose bib" installed.
  • Physical Facilities in Good Repair
    Observation: There is one broken window, along the front wall, in the canned goods basement storage area. Also, the screen associated with the kitchen back door is torn.
    Correction: The broken window, in the canned goods storage area, is a flying insect possible entry (pest control) issue. The broken window needs to be replaced, as soon as possible. Also, the back door screen needs to be replaced or repaired as soon as possible.

  • Hair Restraints - Effectiveness
    Observation: Employees working with food were not wearing the hair covering that we require.
    Correction: We require that all employees, working with food, wear a hat, an open visor, a hairnet, a scarf, or someother type of hair covering that we have approved.
  • Critical: Package Integrity* (repeated violation)
    Observation: There were two large cans of pineapple, with dents in top seams, being stored on a shelf where only canned goods intended for service to customers are supposed to be stored.
    Correction: Cans with dents in top, bottom, or side seams may not be used for service to the public. Cans need to be inspected for dents in seams whenever deliveries arrive from suppliers, and are unpacked. Cans with dents in seams need to be immediately segregated, and placed in a special area , designated with a sign (which may be handwritten) which says "damaged cans only"-until they can be discarded, or returned for reimbursement.
  • Physical Facilities in Good Repair
    Observation: There is paint peeling on the wall b.ehind the sprayer arm assembly-in front of the mecanical dishwasher
    Correction: This area of the wall needs to scraped, and repainted, during the upcoming summer break.
