Correct above items prior to the follow-up. Reviewed employee health with staff. DO not use sterlite shoe containers for food storage. No soap observed connected to the dishmachine - correct in conjunction with 1890. Ice machine deflector and walk in cooler shelves noted in need of cleaning.

    Observation: According to the date mark the pizza sauce (4/7) and mashed potatoes (4/7) should have been previously discarded.

Follow-up inspection for ACR #29926
The comforter previously stored in dry storage has been removed. The interior of the walk in cooler is in need of cleaning. Continue to clean other surfaces in the kitchen as discussed.
Provided a list of phone numbers for County Departments including Family Services and Area Agency on Aging.

No violation noted during this evaluation.
07/11/2013Complaint
ACR 29926
A complaint was received that stated people sleep on the floor in the kitchen of this facility. The complaint also stated that these people also brush their teeth and wash themselves in the kitchen. The complaint further stated that the kitchens are dirty, foods are stored out of temperature, different foods, different foods are stored together, that food is dropped on the floor and that employees don't wash their hands.
EHS spoke with the facility owner - he indicated that he was not aware of the complaint and that there was no one sleeping in the kitchen.
During the inspection EHS observed
- a man sleeping on the floor in the dry storage area. He was told to leave but he left a comforter on the shelf and without using a plumbing fixture
- multiple areas were noted in need of cleaning (interior of walk in, under the equipment and around the floor/wall juncture)
- some items were out of temperature
- meat were stored correctly
- no food was observed to fall on the floor
- the cook and the owner were both observed washing their hands
EHS reviewed 12 VAC 5-421-3000 with PIC.

No violation noted during this evaluation.

Correct the above items prior to the follow-up inspection.
Note: Do not store food in sterilite shoe containers, clean the ice machine deflector.

    Observation: Improper cooling - one and two step.
    Correction: Cool cooked potentially hazardous foods from 135°F to 70°F within two hours and from 70°F to 41°F within four more hours. Cool items prepared from ingredients at ambient temperatures to 41°F within four hours.
  • Critical: Food - Potentially Hazardous Food - Cold Holding*
    Observation: Some foods cold holding at improper temperatures.
    Correction: Cold hold at 41°F or below.
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods (repeated violation)
    Observation: Eggs cooked to order offered on the menu (consumer advisory not posted.)
    Correction: Offer animal foods fully cooked or correctly post the consumer advisory.
  • Critical: Toxics - Storage of Toxic Containers*
    Observation: Bleach stored next to an open bottle of soda.
    Correction: Store chemicals away from food.
