A ROUTINE INSPECTION of your facility was conducted today. Thank you for accompanying me during the inspection as your participation allows me to further clarify processes at your facility that may require further assessment. - Microwave Oven Interior & Door Seals Cleaned Every 24 Hours (corrected on site)
Observation: The (cavity, door seal) of the microwave oven is observed soiled. OBSERVED THE DOORS AND INSIDE OF THE VULCAN OVEN WAS COVERED WITH GREASE, DRIED FOOD AND OTHER DEBRIS.
Correction: The interiors of all microwave ovens including cavities, door seals, floors, walls, and ceilings shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure. ASKED CFM TO CLEAN THE VULCAN OVEN ON A MORE REGULAR BASIS.

Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.DISCUSSED WITH CFM WHO WILL REMOVE THE ICE AND CLEAN AND SANITIZE THE ICE MACHINE.

Observation: The date when the last shellstock from the container is sold or served is not recorded on the tag or label. OBSERVED THAT THERE WAS NO DOCUMENTATION OF WHEN THE SHELLFISH WERE REMOVED FROM THEIR ORIGINAL BAG.
Correction: The identity of the source of shellstock (oysters, clams, mussels) that are sold or served shall be maintained by retaining shellstock tags or labels and marking the tag or label with the date when the last shellstock from the container is sold or served.DISCUSSED WITH OWNER WHO WILL DATE AND RETAIN ALL TAGS ONCE THE LAST ITEM HAS BEEN SOLD.

Observation: The tags for the molluscan shellfish are not being retained for 90 days.OBSERVED A CONTAINER OF COOKED SHELLFISH IN THE BROWN WALK IN REFRIGERATOR. CFM COULD NOT PRODUCE TAGS FOR THE ITEM.
Correction: The identity of the source of shellstock (oysters, clams, mussels) that are sold or served shall be maintained by retaining shellstock tags or labels for 90 calendar days from the date when the last shellstock from the container is sold or served using one of the following methods: 1) an approved record keeping system that keeps the tags or labels in chronological order correlated to the date when the last shellstock from a container are sold or served, 2) preserving source identification by using a record keeping system, and 3) ensuring that shellstock from one tagged or labeled container are not commingled with shellstock from another container with different certification numbers, different harvest dates, or different growing areas as identified on the tag or label before being ordered by the consumer. DISCUSSED WITH OWNER PUBLIC HEALTH SIGNIFICANCE OF RETAINING TAGS AND OWNER TO ENSURE THAT ALL TAGS ARE RETAINED FOR 90 DAYS.

Observation: Observed that the floor tiles near the 3 VAT and 2 VAT sinks are broken/missing and is not maintained in good repair.
Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. ASKED OWNER TO REPAIR THE FLOOR TILES.

- Food Storage / Preventing Contamination
Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. OBSERVED BUCKETS OF FOOD SUCK AS SAUERKRAUT STORED ON THE FLOOR OF THE BROWN WALK IN REFRIGERATOR.
Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.

Observation: The following food item were found subject to contamination after receipt and during storage due to the condition as described: OBSERVED ICE SCOOP STORED OVER CARDBOARD ON TOP OF THE ICE MACHINE. REPAIR LEAK FROM THE HOSE IN THE WALK IN REFRIGERATOR CURRENTLY DRIPPING WATER INTO A PAN ON THE TOP SHELF.
Correction: Food shall be protected from contamination that may result from a factor or source not specified under Subparts 3-301 through 3-306. CORRECTED: CFM REMOVED THE SCOOP FROM THE AREA AND STORED IT CORRECTLY. DISCUSSED WITH CFM TO REPAIR THE HOSE.
- Cutting Boards / Resurface / Discard (repeated violation)
Observation: The cutting boards along the PREPLINE are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
