*Several sanitation and food protection violations observed today. Operator has moved plumbing fixtures and equipment without prior approval. Notice of Violation and 30 Day reinspection. Provide a Chesapeake registered, certified manager on site during all hours of operation (not present today). Conduct and document ongoing training of staff in food safety, personal hygiene, sanitation, temperature control,..... Staff may attend foodhandlers' class at the Ches. Health. Dept. also (class flyer provided).

Hot water available. Clean rear dry storage area of unnecessary items. Develop training logs for employees. Wash temp 120 F Sanitizer level 50 ppm. RECOMMEND FOR PERMIT.

Comments:
No permit issued at this time. Have the facility treated for roaches (several observed in the kitchen today) and reclean floors and drains. Repair water heater to provide 120F water at the dishwasher. Ensure all refrigerators maintain <41 F. Call for reinspection when complete 382-8672.
