The purpose of this visit was to provide a routine inspection. Chlorine sanitizer final rinse dishmachine/wiping cloth bucket: 100/100 ppm. PIC advised damage walls in men's room, resulting from recent urinal replacement, were scheduled to be repaired within the week.

The purpose of today's visit is to conduct a risk factor assessment inspection.
  • Microwave Oven Interior & Door Seals Cleaned Every 24 Hours
    Observation: The cavity of the microwave oven is observed soiled.
    Correction: The interiors of all microwave ovens including cavities, door seals, floors, walls, and ceilings shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure.

Note to manager: EHS observed three roaches in different life stages and highly recommend another pest control service.
Pest control service: Monthly and last service was received 1/15/14

    Observation: Observed raw shell eggs stored at room temperature.---raw eggs at the bar 51F sitting in room temperature is unknown.
    Correction: Shell eggs that have not been treated to destroy Salmonella shall be stored in refrigerated equipment that maintains an ambient temperature of 45°F or less.---Discarded
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following units are damaged: Walk-in cooler, Walk-in freezer
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Critical: Food-Contact Surface in Contact with PHF(TCS) Cleaned Every 4 Hours (corrected on site)
    Observation: Observed knives located at the prep area and cookline that are not being cleaned at least every 4 hours.
    Correction: Food contact surfaces of equipment and utensils must be cleaned and sanitized at least every 4 hours to prevent contamination by bacterial growth.---All knives was cleaned.

    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.

    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: on top of the dish machine, underneath the hot tea machine and prep tables.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required
    Observation: Metal bowls were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.

    Observation: Openings to the exterior of the building are present along the bottom back door. .
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.
  • Removing Dead or Trapped Birds, Insects, Rodents, etc.
    Observation: An accumulation of droppings and dead roaches were found on the walls, ceiling, and under equipment.
    Correction: Remove dead rodents, birds, and insects from the control devices at a frequency necessary to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the walls and floor junction in the kitchen is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

    Observation: Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials.--above the Walk-in cooler
    Correction: Clean intake and exhaust air ducts at regularly intervals to prevent them from becoming sources of contamination.

The purpose of today's visit is to conduct a risk factor assessment.
Note to manager: EHS observed four roaches between the prep table and the wall in the kitchen near the dish area and recommends pest control service.

  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site) (repeated violation)
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.---0ppm
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine.---The sanitizer needed priming.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: Churrasco (Peruvian steak) on menu.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.
  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: equipment shelvings are rusted and outside and inside of walk-in cooler/freezer is not easily cleanable around handle.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the wet wiping cloth bucket with a concentration of 0 ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site) (repeated violation)
    Observation: Observed the handsink by cookline is being used as a dumping station.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site) (repeated violation)
    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests: insect spray - discarded.
    Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination.
