    Observation: The comminuted or ground steak was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria.
    Correction: Continue to cook the meat to heat all parts to 155°F or above for 15 seconds, 145°F for 3 minutes, 150°F for 1 minute, or instantaneously at 158°F.

    Observation: cream cheese cold holding at improper temperatures.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. correction initiated

    Observation: According to the ""consume by"" date on the commercially processed ready-to-eat (RTE) yogurt in the refrigerator, the food should have been discarded 1/25/2016..
    Correction: Discard the food at this time and ensure all commercially processed RTE food is served, sold or discarded by the ""consume by"" date after opening. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: plastic on spray head needs to be removed and sanitizer despiser needs to be fixed in poor repair.

Site visit. Observed floor wet in several areas in the kitchen

    Observation: A condensate drain line servicing ice machine appears to be draining on the floor.
    Correction: Repair/Relocate the condensate drain line to prevent liquid waste discharge onto the floor surface.

Routine inspection done in conjunction with complaint inspection. Drainage from ice/soda machine at drive thru needs attention.
  • Critical: Hands - Preventing Contamination from Hands* (corrected on site)
    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands. (Upon inspection food preparer was wearing one glove, bare hand was touching snack wrap).
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
  • Cloths - Wiping Cloths - Use Limitation (repeated violation)
    Observation: Wiping cloths improperly stored between use. 0ppm sanitizer in wiping cloth bucket.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Critical: Time as a Public Health Control* (corrected on site)
    Observation: Cooked items in holding unit for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time. (Timer was not set for any items in holding unit).
    Correction: Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time.

    Observation: The nonfood contact surface of the ice machine has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of residues.

    Observation: Spray nozzel at 3 basin sink is covered by plastic wrap.
    Correction: Remove plastic. Repair and maintain all plumbing components and fixtures.

    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Cooler was unplugged.-plugged back in during inspection. Food was either relocated or discarded during inspection.

ACR# 32138. Caller indicated she received a dirty plastic bag with the salad she ordered. Stain on inside and very stretched on outside. Appears " as if it came out of the garbage".
Site visit to discuss with Manager and inspect plastic bags being used.
2 boxes of plastic bags in stock and 1 in use hanging at line by the fryers-observed new and clean.
Manager on duty (Amarjeet Brar) was not aware of complaint.
Complaint could not be verified. Health dept contact information provided to Manager in case additional information becomes available.

No violation noted during this evaluation.

Caller indicated observing employee washing face in drive thru (window 2) handsink then serving food on 8/10/13 at 9am.
Discussed with PIC. Handsinks are for handwashing. Please re-educate staff.

Case ID # 30292 - Complain was received stating a whole bug carcass was found in the order of fries of the complainant on 7/13/13.
Observation:
During the inspection no bug/carcass was found on any food item. Four live flies were observed in food prep area. A UV fly trap was observed in the food prep area. Manager on duty (Dee Gordon) was unaware of the complain. Documents were provided to show that the pest control agency was in the establishment on 7/11/13. Manager on duty stated that they would take all necessary measure to control the pest/insects in the establishment.

No violation noted during this evaluation.

    Observation: A food employee continue to wear blue glove after handling raw meats and then used for sandwich preparation. Food handler failed to wash hands, change gloves after touching raw foods and before touching ready to eat foods.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
  • Equipment - Non-Food Contact Surfaces and Utensils (corrected on site)
    Observation: Interior surfaces of the ice machine was observed soiled with accumulations of grime and debris.
    Correction: Clean and sanitize the interior of the ice machine and deflector.
