The purpose of this visit was to conduct a routine inspection emphasizing risk factor assessment.
  • Critical: Air Gap Design Standard, Not Less than 1' (corrected on site)
    Observation: The diameter of the air gap between the water supply inlet and the flood level rim of the mop sink was less than 1 inch.
    Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch. The PIC cut the hose 1 inch above the flood rim to meet requirement.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the kitchen being used for pineapple can drainage.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. Discussed with the Person-in-charge to make the handwashing sink available for handwashing ONLY.

The purpose of this inspection was to investigate a complaint of unsanitary dumpsters, improper solid waste storage, and exterior insect activity. Dumpsters and dumpster area in satisfactory condition at the time of the inspection. No evidence of insect activity was observed. Was advised by PIC that cooperation of several facilities at this shopping center had led to a recent resolution of the dumpster issue. NOTE: PLEASE REMEMBER TO PROVIDE A COPY OF CFM PHOTO ID CARD TO HEALTH DEPARTMENT WITHIN 10 DAYS.

The purpose of this visit was a foodborne illness investigation in conjunction with a routine risk evaluation, emphasizing risk factor assessment. Quaternary ammonium sanitizer in 3-vat sink: 400 ppm.

The purpose of this visit was to conduct a routine inspection. Water heater: Bradford White M2TW75T6BN. Quaternary Ammonia solution used for sanitizing prep surfaces: 250 ppm. 4 dr prep cooler could not maintain 41 degree F ambient temperature in the base of the unit. Top of unit (1st section to receive cooled air) was maintaining required temperature. PIC scheduled service of unit during inspection and will provide copy of invoice to the Health Department within 10 days. Base of unit will not be used for PHF food until the unit is restored to fully functional condition.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site) (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." NOTE: PIC was familiar with policy requirements, but had not had employees sign reporting agreement. Provided short training session and had all employees sign employee reporting agreement.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Breaded chicken topping, sliced pepperoni, and sliced ham topping in 4 dr prep cooler bottom. NOTE: All items relocated to walk-in cooler.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: Plastic cart with broken top.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: 4 dr prep cooler.
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
