A RISK FACTOR INSPECTION of the facility was conducted today. Thank you for accompanying me through the inspection as your participation allows me to clarify information and identify areas where your processes may need further attention or assessment. NOTE: At this inspection EHS discussed Employee Health Policy guidelines with staff and ensuring employees were trained to use hand washing sinks only for hand washing. Staff were observed washing hands between tasks and before wearing gloves. Complainant stated that he ate at this facility yesterday around 4:30 pm with his family. All of his family members are sick with the symptoms of diarrhea stomach ache, loss of appetite especially in the morning. He had ordered whole chicken, French fries, palatine, and water. He further said, he could not go to work, his children missed school due to lack of sleep because of the pain and diarrhea. He (Medical Doctor) or any of his family has not visited the doctor for testing. Cold and Hot hold temperatures of the food items involved in the complain were taken and found to be within range. Also cooking temperatures of Rotisserie chicken were taken and found to be within range. Ambient temperatures of the equipment used for cold and hot holding food were also found to be good. The facility has a complete employee health policy and the CFM stated there were no other reports of illness from either customers or employees. Time was taken to explain proper cooling methods with the Certified Food Manager. Complaint could not be verified. No violation noted during this evaluation. | 04/04/2014 | Complaint | |

PREOPERATIONAL FINAL INSPECTION 1. The installed self-serve soda dispensing machine has not been fitted with "push buttons" or longer actuating levers as was noted on the approved plans. The machine can be fitted with either longer levers, push buttons or be relocated to behind the counter where it is not accessible to customers. 2. There is a large opening between the exhaust hood system and the wall. This opening needs to be sealed. 3. The dumpster has not been provided. 4. Covered trash receptacles have not been provided at rest rooms. 5. Certified food manager has not been provided.
