  • Food - Miscellaneous Sources of Contamination (corrected on site)
    Observation: In the walk-in cooler, plates were being used to scoop food out of big pans. This creates a potential for contamination by hands. Some food was incompletely covered.
    Correction: Protect food from miscellaneous sources of contamination. To dispense food, to move from storage container to food preparation area, use scoops with handles, ladles, tongs, or other utensils appropriate to the type of food, so as to minimize bare-hand contact with foods. (The plates were removed, thank you.)
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site)
    Observation: Assorted items of house-made ready-to-eat (RTE) food in the walk-in refrigeration unit is not properly dated for disposition (chicken, rice, beans, ground meat).
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.     Observation: Employees wearing jewelry (bracelet, watch) on arms and hands while preparing food.
    Correction: Ensure employees are not wearing jewelry (bracelets, watches, wristbands, rings) on their arms and hands while preparing food. This section does not apply to a plain ring such as a wedding band.
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods
    Observation: One menu item (carne asada, "grilled to your liking"), may be served at customer request undercooked meat is provided on the menu without proper disclosure.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived food and identification of the animal-derived food by asterisking (*) it to a footnote which states that this item is, or may be, served raw or undercooked, and that consumption of raw/undercooked meat may increase risk of foodborne illnesses.
  • Warewashing Machines, Sanitizer Level Indicator
    Observation: The warewashing machine is not equipped with a device to warn of low sanitizer level.
    Correction: Alter or replace the warewashing machine so that an audible or visual warning is given when more chemical sanitizer needs to be added.
  • Equipment - Good Repair and Proper Adjustment
    Observation: A panel is missing on the housing for the mechanical parts of the ice machine.
    Correction: Obtain a replacement panel from the manufacturer or install a substitute panel made of material that is smooth, durable, non-absorbent, and easily cleanable (such as plastic).
  • Toilet Rooms - Enclosed
    Observation: Some of the toilet room doors are not tight fitting
    Correction: and/or are not provided with a self- closing door
  • Physical Facilities in Good Repair
    Observation: Some areas of the physical facility need repair or renovation.
    (1) Plug holes in walls, where electrical conduits or plumbing conduits pass through them: (a) below the handwashing sink in ware-washing area

    Correction: and (b) below the soda station in the dining room, in the wall between the soda station and the bar.
    (2) Seal the unfinished (raw) wood blocks supporting the ice machine.
    (3) Clean sides of ramp into walk-in cooler and seal/paint exposed unfinished wood.
    (4) Cover or fill the holes on the interior of the door to walk-in cooler.
    (5) Re-finish the rough wall in corner of dish machine room, behind shelves for clean cook ware.
    (6) Repair the paint which is chipping off the hood over grill.
    (7) Re-finish the floor-wall juncture (replace missing baseboard?)between dishwashing area and cooking area.
    (8) Seal or replace cracked floor tiles in the dish machine area.

The above observations and corrective actions were discussed with the person in charge. These deficiencies are to be corrected within 30 days. The facility is in substantial compliance with the Virginia Food Code 12 VAC 5-421 et seq. A food establishment operating permit is issued on 10/31/2014.
  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: The nonfood contact surface of the unpainted wood blocks under the ice machine is not corrosion resistant, nonabsorbent, and/or smooth.
    Correction: Paint or seal (varnish) the wood blocks. Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
  • Equipment and Utensils - Durability
    Observation: A panel is missing from the motor housing of the ice machine, compromising the operators' ability to keep the machine clean.
    Correction: Install the replacement panel for this appliance when it is received from the manufacturer. (Equipment and utensils must be capable of maintaining their original characteristics in order to be easily cleaned and to prevent contamination of food.)
  • Warewashing Machines, Sanitizer Level Indicator
    Observation: The warewashing machine is not equipped with a device to warn of low sanitizer level.
    Correction: Alter or replace the warewashing machine so that an audible or visual warning is given when more chemical sanitizer needs to be added. The alarm device is already on order and expected within 10 business days.
  • Equipment - Fixed Equipment, Spacing or Sealing
    Observation: The vertical metal panel behind the hand sink in the dish machine's room is not sealed to adjoining equipment or walls.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage and cannot clean behind it.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the wheels of the Blodgett oven has accumulations of grime/debris and/or rust.
    Correction: Clean the wheels and re-paint or seal the metal.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Floor or floor covering in the dry storage/ice machine area is not smooth and easily cleanable (unfinished concrete). Chipped/cracked tiles in front of the dish machine. Ramp into walk-in cooler in need of repair (bare wood on the sides).
    Correction: Clean floor and paint/seal concrete floor in ice machine/dry storage area. Seal or replace tiles in dish machine area. Repair and clean ramp into walk-in cooler.
  • Floor and Wall Junctures, Coved, and Enclosed or Sealed
    Observation: Floor and wall juncture between dish machine room and cooking area is not sealed.
    Correction: Seal floor and wall juncture in rooms where water flush cleaning methods are used.
  • Toilet Rooms - Enclosed
    Observation: Toilet room doors are self-closing but do not close completely/tightly.
    Correction: Provide toilet room doors that close completely on their own. Adjust closure devices as necessary.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Doors are not equipped with self-closing devices and/or are not tight-fitting (light is visible at sides/below doors.
    Correction: Protect the food establishment against the entry of insects and rodents by installing doors that are solid, self-closing, and tight-fitting. Install or adjust self-closing devices/hinges. Install "sweeps" or gaskets along sides and tops/bottoms of doors as needed, so that no light can be seen around the door frame when door is closed.
  • Physical Facilities in Good Repair
    Observation: Hole in wall between bar and beverage station needs to be sealed to fill the void around the pipes/hoses that pass through the wall at this location (under the beverage-station table). Also, the metal sheathing/covering on inside of door of walk-in cooler has holes (or is ripped or torn). Also, there are holes in the wall below the hand sink in the dish machine room. Also, the foam-insulation patches, which are plugging holes that were in the metal wall to right of flat-top grill, protrude from wall. need to be cut off flush to the wall, and sealed/painted to be smooth and easily cleanable. Also, paint is peeling off the front panel of the grill hood (i.e., the panel facing into the room).
    Correction: Patch or fill the hole in the wall between the bar and beverage station, and under the hand sink in the dish machine room. Patch or cover the torn spots on interior of walk-in cooler's door. Cut foam-insulation plugs flush with wall, seal/paint those spots to be smooth and easily cleanable. Remove loose paint on grill hood's front panel and re-paint the panel.
